I'm not sure if Miranda is capable of being the Fire Team Leader. I know a Loyal Jacob and Garrus are, but I'm not sure for her. She could be why Legion died.

Jack died because you didn't have the Hull Upgrade. By the end game you need the Hull Upgrade (From Jacob),the Barrier Upgrade (From Tali)and the Weapons Upgrade (From Garrus). For each one you don't have, a Teammate will die on the way there.

Also, was Miranda loyal or not? If not, that could be the reason why Legion died and why she died after the final fight.

if you upgrade Legion's sniper rifle and equip him with it, he'll survive
(don't know why he would need a sniper rifle in the vents though)

Dammit, I think I want to replay with no loyalty. That ending would be FAR more dramatic when teammates are biting it left and right. Everybody survived on mine:(

Oh dear, my first attempt was a train wreck. A truly sorry, sad and sordid affair! what was I thinking? On the flip-side the Normandy was incredibly spacious and roomy after the showdown.

Oh yes, the problem at hand. Sounds like either a bug or some pre-showdown arguments. Both Legion and Jack are susceptible to unfortunate accidents if you play favorites without the charisma to smooth things over.

I got to ask the topic creator did you have Jacob's upgrade for the ship? If no then that is why Jack died at the beginning. After Miranda's and Jack's loyalty missions did you take Jack's side. If yes that is why you lost Miranda.

Here's why the ship upgrade you didn't get was heavy armor plating for your ship which would have protected Jack from dying. When sending someone in the vent if the specialist or the second fire team leader isn't loyal the specialist will die. and then you mentioned that Garrus and Grunt were your party assuming you took them with you into the final battle it makes perfect sense you lost Miranda. Since Miranda has a less chance of surviving than Jacob and since they both weren't loyal Miranda died.
The order squad members die is as follows: any non-loyal squad members, Mordin, Tali, Kasumi, Jack, Miranda, Jacob, Garrus, Samara/Morinth, Legion, Thane, Zaeed, Grunt

I ran into a bug where everything was peachy until the final cutscene where it just randomly showed mordin dead on the ground... I was pretty pissed.
That happened to me, I'm pretty sure it's not a bug; he was defending that doorway with the others, and when I came back through he was dead.

Unless you're talking about a different scenario?

I was pretty sad though, especially after discovering his musical talents.

Jack was killed by the laser because you forgot the armour upgrade. If you didn't use charm/intimidate to settle the argument between Legion and Tali, whoever you sided with will be disloyal, I made the same mistake on my first playthrough. I'm pretty sure if you use the dialogue option saying you have to hurry before leaving for the final battle, the more time you take the higher the chance will be of party members dying.

I had it like this

1) All ship upgrades
2) All character upgrades
3) Everyone except Miranda's loyalty
4) Legion in the vents
5) Garrus leading both fire teams
6) Jack as the biotic
7) Took Tali and Samara on final fight

Everyone survived.

I lost so many people :( Including Tali, Thane and Grunt(3/4 of my favs), luckily Garrus (1/4 of my favs) survived so I was mildly happy. Damn Miranda just didn't die!

Odd... Must be a bug. Personally I killed Miranda perposly. I hated her character and was glad to see her go.
I had.
Vents: Tali
Fire team leader 1: Jacob
Escort: Grunt
Biotic sheilder: Jack
FTL 2: Jacob
Team for all: Garrus, Samara.
Also I had full upgrades

You could have just sent Jacob through the vents to kill him.

Now to try and answer the other things. Jack died because you didn't get the Jacob's Normandy upgrade. Random people die holding the line if you don't leave enough combat focused characters their as well. Combat characters being Zaeed, Grunt, and Garrus. Random people die whether its Miranda, Mordin, or Tali or someone else. It's not a bug nor a glitch.

Actually, this explains it and goes into more detail then I could here. http://social.bioware.com/forum/1/topic/105/index/2933016

Hehe, that's really funny. Wow, you really hate Jacobs :p and when I did this I managed to save everyone on the first try.

Fire team 1: Garrus
Escort: Grunt
Vents: Tali
Biotic Shields: Samara
Team 2: Thane I think

*Saving everyone so if you don't want to know don't read*

If you didn't do jacob's loyalty mission are you sure you got his ship upgrade? If you didn't then Jack is always the one who dies.

Legion should've survived since Miranda, Garrus and Jacob are the ones you need to have lead the fire team to get through with no casualties.

Holding the line everyone basically has a "Value". The higher their value the less chance of them dying. Mordin, Miranda, Tali and Jack have the lowest values. Garrus, Jacob and Grunt have the highest values. So I guess disloyal jacob has a higher value than loyal miranda.

Mordin has the lowest survival value. He can just up and die if you keep him back to hold off the collectors.

Didn't lose anyone except Zaeed and *gulp* Tali.... just a minute there's something in my eye...
*pats on the shoulder*

Poor Tali :(

I brought her on my team for the entire mission just to make sure she'd be ok... which she was!
Poor Legion bought it closing the door for me.
I had Sammara lead the first fire team, Garrus the next, and then Samarra again (to keep Miranda and Jack from bitching at each other)

Also, I used jack as the bubble shield. I figured she was naff enough at combat but her biotics were really good... Sammara was a logical leadership choice.

Again... Poor Tali :'(
Samara is NOT a valid leadership choice. She is a legendary figure, being a justicar, but in all her missions, she works solo, and has never given tactical orders. The only suitable leaders are Miranda, Jacob, and Garrus.

This happens if you don't tell him to take the survivors. Look, I know its not supposed to kill him, but for some the game wants him to be the guy that takes the survivors, got me as to why.
'"Hold the line!" Personally I prefer to get the job done and go home.'

They put in clues about jobs in peoples dialogue and at the end there are some points where they try and trick you into killing people.
