My first time through I lost Garrus [sub]and Jack[/sub] and I was crushed T_T. Garrus was one of my favorites. In fact, he may be my NO.1 favorite. Needless to say I saved everyone's ass the second time through. EDIT: What I want to know is what ME3 will do if you got the ending where Commander Shepard dies.
You don't import your saved game, and you have to start a game with a prefabricated Shepard, like in ME2. There was a wacky rumor that you will get to play as Joker, but that's pretty well debunked.

I didn't lose anyone. I don't want to lose the opportunity to interact with former squadmates in ME3. Besides, the choices in the suicide mission were pretty straightforward, and not doing a loyalty mission seems to just be not experiencing content you just bought. Seems like such a waste.

well, first ever playthrough i lost mordin, which really egged me off because they needed a tech specialist and who would have thought the only other full engineer/tech character wasn't qualified to run through the vents?! Lame. That was it though.

On my first playthrough I lost Thane and Jack. Thane because I didn't have a specific upgrade (well I was missing about 75% of them) and Jack because I chose Miranda in a fight. On my second one I only lost Miranda because she wasn't loyal. Third playthrough, I'm not losing ANYONE this time.

Did it once and only lost Jack. But think I'll play it again, with ME 1 too on X-box, and lose no-one. She died after we killed the giant human robot thing and the group falls down.

There's ways to kill em off and ways to preserve them.I choose to preserve them. But I did have a problem where Mordin died each time so I had to replay the suicide mission...

Here's the handy flowchart that tells you how to save/kill (if you're into that) the individual members of your crew.

Miranda gets a free pass through a lot of stuff...

Nobody on my first play through. On my second I decided to get all loyal and no upgrades (Kasumi, Thane, and Jack died on board the Normandy.) On my third I got all upgrades but no loyal (everyone but Grunt, Miranda, Zaeed, and Jacob died.) and on my last play through I made sure Jack died (I hate her as a character, the only character in a Bioware game that I cannot stand at all.)

Also there's been plays where I purposefully kill Samara (She threatens to kill you after the mission if you're Renegade, WASN'T HAVING THAT!) or Garrus/Tali (just to see what I end up with in ME3, i'll probably have Liara, Kaiden, and the new guy and thats it :/ but oh well.)

I did 3 playthroughs, one with everyone, one in which I lost Jacob and Legion, and one in which I lost Tali, Jack, Grunt and Thane....

I like to mix things up.

When I watched my friend play, he lost Mordin at the very end, and we didn't know why.

So when I played, I did all the loyalty missions and got all the upgrades, and then once I got to the end, I took Garrus and Mordin with me, because I thought if I took him with me, maybe nobody would die.

I was wrong.

Tali died. And I died a little inside :(

For the record, I sent Kasumi into the pipe to do the hacking, I made Miranda the leader of the squad both times, and I sent Grunt back with the survivors. I thought I made good choices, everyone did their jobs correctly (I forgot to activate Legion, and then if I activated him and did his loyalty mission, my crew would have died).

Jacob volunteered, he died
Jack and Legion died before we even landed
Grunt died from Seeker Swarms
Miranda and(I think) Mordin died defending.

I only bought the ship guns upgrade and did about 5 Loyalty missions before I got sick of the Loyalty stuff.

I think that leaves Tali, Garrus, Thane, and Samara left in my crew along with the Normandy crew.
I don't understand people like you. The loyalty missions are literally the other half of the game. Why would you purposely not do half of the game and just screw yourself out of content you paid for?
It was my decision, plus without people like me this thread would cease to exist as everybody would survive the mission every single time.

And if you don't understand me, I could throw that back at you(your avatar), but I won't

And I went back through the game a few more times to get the acheivement so its not like I missed anything.

I let no one die.
Despite the fact that I hate Jack and Miranda with a burning passion, I did not let them die.

During my first suicide mission only Legion died (he was specialist), because, as I found out, Samara is a crappy fire squad leader lol. Oh, and Kelly now works as a milkshake.
happened to me too

During my playthough, no one died.

For the record: I made all the loyalty missions and fully upgrade the ship. Kasumi did the hacking on the pipes, Jacob was the leader of the fire squad the first time and Miranda the second, sent Garrus to escort the survivors and my team during the final fight was Samara and Tali.

my first playthrough, i only lost Jacob, and it was simply because i was in an XBL party and wasnt paying attention the beginning plan that they were coming up with. i picked him to do the crawling through the pipes or whatever... the job that you have to help them with by lowering the heat inside the pipe, and opening doors so they can continue.

well, he got shot in the head trying to help close the door. i was irritated... Kelly also died, but shes not a squad mate. still part a of the crew, though.

and on my second playthrough, i lost nobody. BUT, Jacob took a shot to the stomache and started stumbling. i was seriously like "Jacob, if you screw me on this again, I SWEAR TO GOD..." then he shook it off like "I'm aight, dawg." (lol racist humor)

one of my friends on live lost like 7 squad mates, though lol

if you want to avoid losing all your teammates, just do every loyalty mission, purchase ALL ship upgrades (excluding the health bay scar removal machine), and make logical choices during the suicide mission.

legion for the tech mission
miranda to lead the second squad
samara to provide the biotic shield

those were my choices for a perfect mission.

also, if you want kelly and the other crew members to survive, you need to start the suicide mission as soon as possible, after you are finished with legions suicide mission (seeing as he is the last member you recruit)

good luck on future playthroughs

None, and on the very first try as well.

Legion took care of the tech pipe, Miranda took care of the squad heading for the ship, Garrus took the fire team, and Samara (or possibly Jack, though I'm 99% sure it was Samara) covered us with the shield.

Zero. No-one. First playthrough (Paragon MaleShep), I refused to let anyone die, and so no-one dead. Second playthrough (Paragon-ish FemShep), I actually didn't have everyone's loyalty (was missing Jack's)...But tactical deployment of the squad meant that, again, no-one left behind.

...I really have to question how you lost that many people, unless you were intending to.

Edit: Oh, and as only half the people giving "advice" seem to have gotten it right.

Upgrades: All three.
Tech: Kasumi, Tali, or Legion.
Both fire team leaders: Miranda, Jacob, or Garrus.
Biotic Shield: Samara or Jack.
Crew Escort: Any, reccomended Mordin as he's bad at...
Hold the Line: Make sure you leave Garrus, Grunt, and Zaeed, as they give the best chance of no casualties there.
Bossfight: Any.

These -all- require loyalty. Loyal squadmates have a better "hold the line" score too.

None. They all survived on both playthroughs.

You just need to make sure to solve their daddy issues and pick team members that make sense for the specific tasks. I.E. Legion or Tali for the tech specialist, Miranda or Garrus for the second FT leader, Samara or Jack for the biotic. As long as you have their loyalty everything's golden.

I looked it up after "refusing to let anyione die" and someone still died at the closing of door. You need to have gone all the way in paragon/renegade to not loose someone apparantly. So me picking different "moral" actions screwed me over for no good reason in the end.

And it annoys me hearing people say they "refused to let anyone die" when the game doesnt care when it comes to it.
