Then there was the Macho Man Randy Savage dragon skin mod....

Mods that went horribly wrong?

I'd say probably 90% of all the mods out there in the modding community; across all games/platforms. (some people just can't program well)

Also, roughly 50% of all mods I got through Desura early on. Things were...inconsistent, to say the least.

On a personal note, one of the worst mods I ever came across was one for Half-Life 2 that replaced every character model, save for the key players (Alyx, Barney, Eli, etc, etc), with one of the civilian character models. The same model; for all NPCs.

It also changed their behavior patterns so that, no matter if they were originally Combine, zombie, headcrab, human, or whatever, they would attack the player as soon as they were within sight range.

Worse still? Their mode of attack was as follows:

They would turn to face the player.
Their eyes would bug out.
They would move into a half-run/half-walk shuffle towards the player.
They would immediately start chanting, "Hey. Hey. Hey. Hey.", over and over in a monotone.
Once in range, they would attempt to punch the player to death.

It was one of the dumbest mods I've ever seen, but oh the hilarious nightmares it spawned. Especially seeing one of those things in an air vent, crawling towards me (by crawling, I mean shuffling oddly on it's belly in a very bugged out animation), chanting, "Hey.". Or, seeing a group of them rise up out of the sand.
