In 11th grade me and my friend were discussing computer hardware and he tried to convince me that the motherboard and the hard drive were the same thing...I still facepalm about that
Never had a facepalm moment in a game. I only get those when I watch commercials or when an adult laughs at anything in a Pixar cartoon, or when Robbin Williams tries to be funny. Yes, American humor is completely lost on me. & why are horror movies so funny?

There was the time when my sister said (And this was back when the Veronicas were big) something about how the new Veronicas song was so great and when I told her that It's a long way to the top was written and performed first by AC/DC she shrugged off and said that their version was far superior.
There was that case where some die hard Taylor Lautner fan wrote to one of the guys behind the wolfman remake saying how much of a ripoff the film was and how "Everyone knows that Stephanie Meyer created werewolves".
There's all these emails the bloke behind the very funny website the oatmeal has received.
There are the times when I've accidentally purchased bolt-action sniper rifles in Counter strike's (Source) zombie mod servers.

The ending of Neon genesis evangelion. I just watched it and the reason I'm facepalming is not because it was bad but because I don't get it and now I feel stupid. Urgh. I'm too tired.
A conversation I had with a friend a while back.
Me: So, I'll call you tomorrow morning, okay?

Her: Hmm.. That sounds a bit early. How about 7:45?

I was a jury member for a coroners court, where a death is brought before us to see whether it warrants further investigation or not... they are generally accidents due to infrastructure flaws or something that happens in a public place thats no ones fault.
The Sergeant was explaining the circumstances of one case to us before court where a cyclist was run over by a truck because her bike hit a pothole and she fell under the wheel, when a jury member asked 'was she alright?'
Silence from the Policeman, facepalm from the rest of the room.

When I saw the commercial for The Bachelor:The Video Game in the movie Valentines Day. I stayed in facepalm position for at least 5 minutes after hearing that you can "sabotage your opponets" and "experience the fun of flirting and the drama of dating"
