Fast headcrab Zombies. Or, actually, just Ravenholm in general. I was out of ammo, and had to fight off 3 with the crowbar. And then the flashlight ran out.

The flood as well. Getting lost in a complex with infinitely spawning parasitic octupi and zombies is NOT fun.

Easily Bowser's clown copter thing in Super Mario World, and
more easily the redead in Zelda: Ocarina of Time. I still get the chills from those things.

I recall bringing that movie to my preschool to watch and nearly all 30+ of the kids cried and left the room when ET came on screen except me and like 2 other kids. I guess ET is - with those formative years behind me - really just a benevolent sausage monster with a heart of gold.

Oh and video games? The bad guy from police quest 3 scared me. He was a satanic serial killer who stabbed your pregnant wife. Kinda heady stuff for a 10-year old.

I openly admit that Fast Zombies scare the shit out of me, but I for one can't stand the screams of the normal ones when you set them on fire, it just creeps me out so much.

Dude, the Blob. That thing scared the SHIT out of me, especially the part where it fell on the guy and ate him like acid.

And the GET OUT sound from Donkey Kong 64. Not a monster, but really really wetyourpants scary.

Since PH was already mentioned....

The Split Lizard from SH1
The final boss of SH3
The big attached to the wall thing in the final battle at SH4
The Fog and darkness from pretty much every SH game.

If you consider the whole town as a monster, then include Bleeding walls, rusty metal gratings and static noise to that list.

I can never figure out what exactly they're saying (yeah, they're saying something) but I'm pretty sure one of the things they say is "Oh no, not again, no no nooo" or something along those lines. And "my face, my face!"

The first time I ran into the Flood scared me because they were so creepy and appeared with so little warning... especially the first time one stood back up after I'd shot it dead.

The Berzerkers in Gears frightened me a bit too, as did the Big Daddies in BioShock.

Of course, after beating all the above repeatedly the scare wears off. Unlike the scare a Predator or xenomorph still inspires from my time in the FPS Aliens vs Predator... *shiver*

I can relate to the Flood, but I think my reaction was more panic than fear.
I met them first on CoOp with my best mate, and we were running around like mad trying to avoid all the small headcrab thingies.

When I was a kid I played a game called Alien Trilogy (at least I think) The game started with you in an elevator and when you stepped out you were in a corrider.
There was a Facehugger in that corridor.
I knew it was out there.
I would literally start up that game, but just REFUSE to leave the safety of my elevator :p
Eventually my uncle would help me kill ze nasty Facehugger.

I'm about 10 years older now so it probably won't have the same effect on me :p

When I was about 5 (over 20 years ago) we borrowed a computer, tape drive, monochrome monitor, it was awesome. The only thing it came with was a game called Transylvania, which, I have to say, was pretty well done for the time. Anyway, there's this bit where you have to run away from a werewolf chasing you, and we could never do it and always died when the damn thing popped up in front of you and ate you. I got so scared that I never went anywhere in the dark by myself :(

More recently.. Ravenholm (the first time through) was... unnerving. Also poison headcrabs and Fast Zombies... I hate the way they move at normal speed and then suddenly leap at you unexpectedly.

Oh, and the first time I saw a Big Daddy, after it left I was scared I was going to bump into the thing after the cage opened. Actually, I'm surprised no one has mentioned Big Daddy yet... surely I'm not the only one who was a little bit nervous about them when they first make an appearance...

In Games, i was afraid of Dogs and Giant Spiders. In real life, not so much (...although i never met a giant spider, i'm referring to regular sized ones). Also the Alien from Aliens.

Nowadays, Poison Headcrabs freak me out. Also, Facehuggers...Screw the Monster under the Bed, Facehuggers are the real deal (Yes...i know they are not real...but they still freak me out)

The only game that really freaked me out was the first Silent Hill. The later ones, I kinda knew what was coming. But the first one... the start of the game, before you enounter any monsters. The town is just so eerie and foreboding. And then you find that radio that makes weird static. And then stumble into an alley. Wait... why is there is a hospital bed here? Why is that wheelchair... but.. AAH! GET ME OUT OF HERE!
Still the best horror game I've played.
