Games with over the shoulder shooting where you can't switch the shoulder it's... over. Especiall in Mercs 2. Oh, and games where you use L2, R2, to aim and shoot, and can't change. I like shooting with my trigger finger, damnit!. Shooting with R2 even gets people killed, as I often put my controller on the ground for cutscenes, when the game starts, I pick up the controller and the button gets pushed slightly and kills a teammate with a headshot.

Ahhh... Men in Black. I remember I rented it, played it for about 15 minutes before returning it to the rental shop demanding a refund or a replacement game that wasn't so awful. (well I personally didn't because I was like 8 but I made a fuss to my dad so he did it for me. haha)

The one that gets me the most is in close quarters games where you're only real attack is X,X,X.
Yeah,never play Eat Lead:The Return of Matt Hazard. The rest of the game was alright but the melee attack was literally,unashamedly X,X,X.

OT,The Learning To Fly levels from GTA:SA. To this day I have never completed SA because I've never been able to complete the learning to fly missions!

Most annoying game mechanic for me would have to be trial and error gameplay, where the only way to succeed is to fail over and over until you learn the level by heart.

Textbook example - Tomb Raider II, the Wreck of the Maria Doria level. Collaspable floor clearly in view. 'Ah ha', you think, 'I shall run across quickly so I do not die!' So you run across quickly, and are immediately killed by a set of barrels that roll from off-screen and kill you.

In Tomb Raider 4, you have to drive a jeep over some sand dunes. However on the other side of the dunes are holes in the ground which will cause you to die should you drive into one. Except you can't see where the holes are because the sand dunes obscure your view, and once you drive over the dune and see the hole in front of you it's too late.

Those instances really pissed me off.

And getting paralysed in Oblivion.

Who the fuck at Bethesda thought those mechanics would be in any way fun?

I hope they fix lots of thngs in the next game... the combat, the stealth, the horses, the repetition...

This one has got to be high on any people that play spy in TF2.

Firstly (Not Annoying); A pyro's flamethrower will set you on fire, cloaked or not, and a person on fire will be seen, cloaked or not. This is fair and balancing.

Secondly (The Annoying part); Those god damned pyro's that camp dispensers and just perma-flame. They seriously spend 2-5 minutes looking around spasticly and holding mouse-1. This makes your life very, VERY difficult.
