Neeshka from NWN2. Get out you screechy yank.
Oh god yes. Never has a character made such a compelling case for being punched in the face. That angel guy in Planescape: Torment? Weaksauce. That one guy in Mass Effect? Weaksauce. Reporter ***** from ME2? Weaksauce.

None compare to the OHAI I'M NEEEEEEEEEEEEEEEESHKA.

Must... punch... face...
Howdeh, MAH NAIMES NEESHKUH, an' ain't this here ye olde fantasy laind juuust chaaaarmin' huh?!?!

I think Jack yells that from Mass Effect
That was it!! Well - partly anyway. It wasn't Jack (I've yet to play the 2nd one) but the yells were from Mass Effect 1. Even my vague recollection was a bit off; they weren't from party members but enemies... I don't know if that makes it more or less annoying given that I killed everyone who yelled that.

...So I have that to look forward to with Mass Effect 2... guess who is NEVER going to come along on missions! :D

Thanks! :)
I don't remember her saying it much, and i dragged her around everywhere on several playthroughs. Odd how much of her combat dialogue i can remember.

But the real reason to leave her behind is her power set. What a fucking joke.

Amata from Fallout3. Her character is a self-important **** in itself, but the voice - while the actor isn't bad - is impossibly annoying. Nothing a shotgun to the face didn't cure.

All English VA's from BlazBlue.
Are they really that bad? I thought Ragna, Relius and especially Tager sounded awesome in English. I felt it was more the dialogue itself that was the issue.
How's someone voicing Taokaka meant to give a good performance when they have to squeeze in like three "meows" per sentence?

As for me, I have to go with Xiaoyu's English voice in Street Fighter X Tekken. It's kind of a cheating answer because I already dislike most roles Carrie Savage has done, but the horrible, guttural screeching sounds she makes during the super arts genuinely hurt my ears.
I can't find a good quality video of the super art in English to prove it, but you're probably better off not hearing it.

Tidus. Goddamn Tidus, I hate him, his voice, his laugh, his stupid face and his entire stupid blitzball moronic character.

one time, when I was playing star trek online, I got a bug where whenever another sound was played, a guy's line would repeat. even though he was half a map away.

"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"
"I don't like Hassan anymore than you do"

SHUT IT.

The "I will destroy you!" gained notoriety from ME1 (arrow to the knee style) because it was shouted by enemy female biotics continually. The fact Jack says it in ME2 is as a joke hinting towards the irritability of the phrase in the first game. Someone else says it in ME3 too.

I think she's designed for easier difficulty settings. As soon as husks get armour, she's a waste of time.

Rosh Penin, Jedi Knight Academy.

Oh my god I was so glad I got to hurt him. And then I was outraged when I fired up Skyrim, only for someone voiced by the same guy to start delivering me letters.

So much rage.

Harley Quinn. Batman Arkham Asylum/City (haven't played Origins yet, so maybe there too). She just has the most harsh, high pitched, discordant voice I've heard in a game. The same in Batman: The Animated Series.

Also, if we're not limiting this to just videogames, Iago from Aladdin has a voice perfect for silent film.

Tidus from FFX. Liked the game mostly, but dear God. Him and Rikuu should have been recast and the characters would have been better.

Sam Hoffman and Captain Montressor from State of Decay are really annoying. That's really all I've got.

Izsha from Heart of the Swarm. Talking in a monotone high register for the whole 12 hour campaign is probably not a good idea for a voice characterization. I also didn't like Dehaka either, he pauses while delivering a sentence more than Shatner.

Any character played by Wendee Lee. That voice... I just can't stand it. I see her more in video game roles, but yeah. Bout the only one I can stand is her in Cowboy Bebop.

...for the life of me I cannot think of what game it is I'm thinking of 0.0 Something I played recently... not necessarily a recent game, just to point out. One of your female companions (AI controlled) kept on screaming very loudly in a savage way;

I will destroy you!!!

over and over and over again... generic line I know. Nothing inherently wrong with it, the line or her delivery of it. It's just the fact that she felt the need to scream it at least every 8 seconds. For about 20 minutes.

This is gonna bug me all day now - what game was it!? I'm pretty sure there were guns involved... it seems I have blocked it from memory.
I think that's Mass Effect, the first one. The male voices for that same line for the nutso people you fight were just as grating.

Sonic.

Because it's no longer the 90s yet his character still sounds incredibly fucking annoying, aka, "cool" for the 90s.

Also Farengar to a degree.

"Hchave you come to dischush the ongoing hoshtilities, like the rescht of the great warriorsch?"

WE GET IT, IT'S SUPPOSED TO BE LIKE NORWAY, BUT JEEZ.

EDIT: Honorable mentions go to Indrick Boreale and the narrator from Dawn of War.

But they don't count as annoying, just plain silly:

I've got two old ones.

1. Willow Rosenberg from Buffy: Chaos Bleeds. In the first game Alyson Hannigan did the voice of her own character, but in the sequel she wasn't available. The problem was the voice actress they used was trying too hard to sound like Alyson and it didn't sound natural. The Buffy voice actress did a much better job sounding like Sarah.

2. Princess Daphne from Dragons Lair. I finally got to the last room, but her voice was so annoying I just wanted to let the dragon kill me just to spare my ears the pain and suffering of listening to her.

I started playing Borderlands 2 recently, and I can not fucking STAND CL4P-TP. I hate that little robot more than most other video game characters I've ever disliked, and certainly more than is rational. I absolutely despise that you have to stick with him and can't just shoot him or abandon him with the bullymongs/bandits.

I've heard a lot of people bag on Fuuka's English voice, but I never saw what was that bad about it. Maybe the lines were a bit 'Captain Obvious' and certainly repetitive, but that's more of a problem with how the lines are programmed, not the acting.
Maybe it was the puns, but Teddie's commentary bothered me more than Fuuka's ever did...
