Fallout: New Vegas- A game breaking glitch that caused the game to freeze whenever I entered the New Vegas Strip. The fix was quite random and made little sense to me. You had to wear a old cowboy hat whenever you entered the strip. It worked just fine after that.
That game would freeze up if you looked at it wrong. That game has to be the most annoying glitch wise... or at least it was if you played it within a year or two of launch.
Actually, the PS3 version got worse the more they patched it. The final version with all DLC installed was unplayable. If you went to Freeside, Khan's Encampment, or the Stripe, you would drop to 2 FPS, You had to somehow save then turn your console off and start the game up again.
I have the ps3 version, started playing again recently and had far fewer problems. Was thinking about dlc (at least the one with Dr Venture) but thanks for the warning. When was the last time you played it? I think they patched it again within the past year or so.
I thought it got unplayable to begin with, it was why I stopped playing it. There were a couple of areas like that where it would freeze up no matter what, like the deathclaw quarry.

They might have. You could probably go check over at fallout wiki or something. I turned my game in a year and a half ago, so I wouldn't know. Also the DLC you are talking about is the only really good one. It's called Old World Blues. Honest Hearts isn't that bad, but skip the other two.

I think that by a fairly wide margin, the funniest compilation of glitches and bewildering encounters I've ever seen is still 'Wild Wild Wasteland' by Criken. It's pieced together from playtime in the infamously buggy Fallout: New Vegas:

That game annoyed the hell out of my with its crashes and glitches until I fixed it up with the unofficial patch. It was hilarity from that point onward. I think I used to run into the glitch where someone would be composed of nothing more than free-floating 'gib' parts like eyeball and limb chunks fairly often.
