Ah, what's a "Most Disturbing" thread without Silent Hill 2? In this case, it's the scene in which we see...uhm, how much of a lady's man Pyramid Head is. I'd put up a link to it, but it's kinda NSFW.
The sound he makes... THE SOUND!

In Dead space where the womans head explodes. I was like "WTF...." and backed the fuck away.

F.E.A.R had a few aswell.
I think the people who hadn't been turned really added to the atmosphere of Dead Space, particularly the guy in the Medical level. What also got me was the level where you hear this in the background-

Amnesia: The Dark Descent.
THE ENTIRE FREAKING GAME

Also, falling into lava and losing all your diamonds in minecraft.
Just generally being unprepared for zombies in Minecraft is frightening. 30 kilometers underground, only 3 mining picks and 2 torches with a ton of stone, then you hear the sounds in the walls.....PSHHHHHHHHHHHH *boom*
Judging from the sound effect, I assume you meant Creepers. Either way, I agree. The only time that Minecraft has made me actually scream is turning around into a creeper.
In minecraft, I was standing outside at night (a dumb thing I know, but I thought I had lit up the area enough), and I heard an odd, but familiar noise. I turn around, and there were THREE creepers in my face. Boom. D:

Competing in the Man Festival in Shadow Hearts: Covenant and finding out that the punishment for losing is:

a buggering

Then discovering the the prize for winning is:

a buggering

You're buggered if you win... you're buggered if you lose... but you only find out when it's too late to back out of the Man Festival, so you're going to be buggered no matter what happens.

It certainly is a unforgettable moment in gaming, but it's also pretty damn disturbing because, no matter how much they try to cover it up with euphemism, funny music and comedy sound effects, the underlying tone is pretty sinister and dark, especially as the punishment or congratulatory buggering takes place in front of the rest of your party (possibly including the 14 year old daughter of the Tsar of Russia, Princess Anastasia Romanov).

If you play as a Dalish Elf in Dragon Age, there's a very unpleasant surprise waiting for you about midway through the game. That and the whole Broodmother thing. Gah.
Could you PM me that? I played through as a Dalish, and I certainly dont remember anything that bad. Although learning about "The Wardens Curse" was a shock.
Playing as a Male City elf and finding out what happened to Shianni when she was abducted from the wedding was a little disturbing to me.

That God Damn Arl's son raped Shianni.

I was happy when I got to kill him, which turned out to be the most bad ass one ever. I was a dual swords user and I got the lucky kill animation where you stab him in the stomach with the left sword, then cut of his head with the right sword. It was like a moment from Highlander. I almost cried out: "THERE CAN ONLY BE ONE YOU *****!" That was a good moment for me.

I got the animation where you decapitate him with a backhanded spin strike. It was pretty great. That guy was a dick.

That was the only downside I had with the Dragon Age: Origins. Was the Kill animations were completely random. Usually it boiled down to when you wanted one to happen... you just slashed him. I wished all killing blows ended with an animation.

Well one scene comes to mind, although I've experienced many such moments...

THIS:
<youtube=tkYtflr1KsM>

Anybody who played Silent Hill 3 remember that store room well...
That was the scariest fucking moment I'd ever experienced in a game. I'm so glad I got to play through it without being told about it beforehand because that shit was downright masterful.

Though not in the same vein as the quoted text, the scene in Metal Gear Solid 3 in which Naked Snake is taken captive and tortured kept me on the edge of my seat. My asshole definitely puckered when the Boss approached Snake's eye with the knife.

EDIT:

I got ninja'd on that? Sweet.

It wasn't a scripted moment, but i was playing S.T.A.L.K.E.R: Shadow of Chernobyl, and i was hunting around this old abandoned army base for cool stuff. I had the Oblivion Lost mod, one of it's features is that there are random blow-outs (huge releases of radiation) every so often. So i'm skulking around and i hear this huge blast.
I run outside, and the sky is blood-red with swirling clouds and lightning all over the place. I look in my inventory and i have no radiation medecine. I start to panic and sprint to a rusted out bus in a field. I dive inside and crawl my way right to the back of the bus. You can be protected from the radiation of a blow-out if you get under solid cover, but a rusty bus with no windows or doors was hardly secure.
So i'm cowering at the back of the bus as thunder shakes my headphones and radiation starts to drain my health bar, i'm staring at the rusty floor because i'm literally in a blind panic, and i hear this faint rustling over the sound of the blow-out.
I look out of the window of the wrecked bus and i see a figure staring at me through the blurred atmospheric effects. It has hugely long arms, and it scoots around using them as extra legs. (Oblivion Lost also adds new mutants).
So now i'm trapped in the bus with only one exit, with the blow-out draining my health and defeaning me as a long-arm scouts around trying to sniff me out.
Now normally anything that is outside while there is a blow-out is killed, but apparantly this mutant had bugged or some shit.
So after a few minutes the blow-out ends, and i am miraculously still alive, although my radiation bar is high and my health bar is low, so unless i can find medecine i'm dead.
I step out of the bus, and the mutant is RIGHT IN MY FACE. I julmped out of my seat as he smashed my head in.

I have a few stories like that from various games, non-scripted, random events are always cooler.
Stalker was always cool like that.
