And while it's tempting to be completely unoriginal by simply leaving a link, I will have to nominate almost all the music in Silent Hill. If it can even be called music...

I guess this hasn't been mentioned because it's simply too horrifying to remember, but for me it's far too horrifying to forget.

Zelda or Metroid when you're down to almost no life.. if you're in a dangerous place, you know you're going to die. If you're in a safe place but can't find a health up or heart it's just insanely annoying.

Also, the Silent Hill horn/siren that sounds as the world changes. *shudder*

And before this thread goes to the second page, let's not forget the 'PEW!' sound that is made when your computer overheats and shuts down. My PC has a overheating problem, you see. It can play DA:O for about two hours and Sims 3 for ten minutes. *Sigh..*

I almost forgot the dread all of these sounds used to instill in me. Now it's an entirely different reaction. After I hit 17 or so it changed from "No, not another one, where is it?" to "Where is this bastard, I'm gonna send him back to hell!".

-The sounds of the Hunter from Dead Space during the bits where it's stalking you. That's the only enemy in the game that actually scares me without having to use the cheap startle method.

-The sound of the Chameleon running towards you in Resistance 2.

-Barking dogs in Call of Duty 4.

-The string-plucking sound from missing notes in Rock Band.

-The sounds made by both the fast and poison headcrab zombies and the grunting of the antlion guards in Half-Life 2.

A series of loud clicks from a hard drive followed by a trip to Best Buy and a complete reinstallation of the operating system and all my games.

Protip: Don't buy Maxtor drives. I've owned two and they both failed spectacularly.

Not much gets to me but the most dreaded sounds for me would be hearing the hunters and lickers in Resident evil. Also the tank in l4d expert mode really. At least one person always says "ah hell" or worse.
