First time I played a TF2 map with a rail track in it. I was looking down the track going "hey, wonder when the train is going to - SPLAT" Train came from behind.

When using a mod in Fallout New Vegas that changed some animations, it suddently failed, so I was left humping the floor when a deathclaw attacked, and the position was so well placed that it seemed that I was being buttfucked to dead.

I was playing Dead Rising 2 and got to the part where you have to enter the underground. I had one health square left and had already taken damage to it. I jumped off the second to last step on a flight of stairs...and died from a 10 inch fall >_<

I was playing Soul Calibur IV today, as Talim (well, a custom character using Talim's style) and I did a really cool move where you leap backwards through the air, and I landed with one toe on the edge of the stage, teetered for a split second, and fell off backwards. I'm glad I was on my own, even though it was bloody hilarious.

That's more embarrassing for the programmers than for you!

I killed five deathclaws in a row in new vegas, only to die from a radroach pack a couple minutes later.
You can die from Radroachs? I didn't even know that was possible.

Anyway mine would have to be while playing Super Mario, I was REALLY tired and a it open to suggestion when one of my sister's friends said you could kill a thwomp. I don't need to tell you how he told me to do it because every single one of you reading this knows that YOU CANNOT KILL A THWOMP.

Most embarrassing, I wouldn't say it's embarrassing really but in Saints Row 2 I accumulated a hella tonne of Police chasing me so I decided to stop and take them all on... continuously, wave after wave and finally, after taking heavy damage, I got to a point where there were no police in the area for a moment so I decided to find a new spot. Just to turn around and get a good beating by some old man that snuck up behind me. Stupid old man.

Battlefield Heroes. Fell onto an idle jeep and died. I was like "WTF, how is it possible to just fall from a roof onto a car and die?"

Also had a silly one on Tony Hawk's 2 (can't really die on there, but you can get those embarrassing moments.) Grinded a rail at the NYC level, but lost balance, cracked my skater's fruits and fell into the water. So funny that i still do it after around 10 years of doing it.

Operation flash point 1985,

AI: "OH no 4... is down, Damn 3.. is down.."
Me: WTH? oh jesus! SPLAT!

translation:
Me and half my squad flattened at the bottom of the hill - by the Blackhawk that we parked on top of the hill, but clearly forgot to put the hand brake on.
