Anything that can attack you under water. Like the sharks in the 2nd level of the new tomb raider. totally wasn't expecting them, threw my controller 2 feet in the air

I would have to say snipers...the fact that just taking one step out from cover can (and often will) result in my head being reduced to a bullet riddled pasty is one reason, the other being that you often don't know where the bastard is and you can't strike back with even a reasonable chance of success unless (drum roll here please) you are a sniper.

The regenerating necromorph from Dead Space. It's easy to handle at first, but once you're on impossible mode, you run out of ammo, and this son of a ***** jumps out of a nearby vent, it's another story. I fucking hate that thing.
