For the game that has caused the biggest frustration for me would be MW2. The amazing searching for new host thing is one of the least working systems I've seen in a while. It first spends 20 seconds searching for a new host. After that it starts to lag. After a minute, it kicks in again, takes another 20 seconds and maybe works. Add that with the crappy spawn system and you've got yourself with a very happy gamer.

Also, any music game can get really frustrating really fast. It took me 7-8 times to get through Green Grass and High Tides in RB1. The song is around ten minutes long. I always failed around 8 minutes.

Fuck saving helpless hostages with friendly fire on in a room full of endlessly respawning half tank half man enemies who have full body armour plus halberds and instant death traps and archers and... fuck.

I'd also like to un-nominate Dead Rising. The save system was designed to give the player a sense of responsibility and consequence upon death, and saving never takes any more than a few minutes.

Having said that, I can completely understand the frustrations with it, including feeling rushed by the six hour time limit, but it was designed that way. The sequel will at least have three save slots and co-op, so perhaps some people who were put off from the first may have a go at the second.
