Most Infuriating And Difficult Achievement/Trophy/Level in a game?

This brings back memories of when I was trying to get the "Mile High Club" achievement from Call of Duty 4. This is one the the most annoying achievements I've ever gotten that comes to mind. I hated the mission and achievement a hell of alot, but felt compelled to get it, through some strange sense of incompletion of the game. Which achievement/trophy/game level was it for you?

P.S Please don't message me telling me how sad and crap I am because you got the achievement first time. I highly doubt that you did, and it's not the point of this post.

I have to go with the Yahtzee's Duke Nukem Forever achievement of going outside, meeting a nice girl, going around the world in a sailing boat and having three blonde children.

(bioshock) well the brass balls achievement is infuriatingly difficult. Get through the entire game without dieing once your kidding right. I found it hard trying to stay alive on normal difficulty.
the seriously 2.0 achievement is a bugger to complete. 1,000,000 kills is an insanly large amount of kills to get on gears of war 2

Probably "Beans Bullets Bandages" in Battlefield: Bad Company, which is get 10002 total kills online. Another one would have to be the "Full Deck" Patch, which you get by getting 52 kills in a single round.

Little rocket man from Half Life 2 Ep. 2. Had to carry that thing around for the entire game, and it ruined the experience.
Darwin's parachute from Battlefield Bad Company. It's easy to get, but it's nearly impossible to get without looking on the forums (because the description is wrong).

Try saving the little sisters instead of killing them. You get an ability on the 3rd one that lets you control a big daddy. Use the big daddy to fight other big daddies so that the one you control kills the one with a little sister and the one you were controlling is injured and easy to kill.

I have good news (or bad?), if you turn off Vita chambers and save a lot it's actually a breeze.

Heck, any of the "make 6 friends" achievements for that matter. I defy any one of you to attempt them. I can assure you boredom and suffering if you do.

It's in the options, my friend, just be sure to start a new game when you do this, since using one at any point "taints" your existing saves.

Battlefield Bad Company - I am Bad Company! Get EVERY multiplayer award possible. This includes 52 kills in a match and getting 5 (I think) boat kills in a round. I think it's the only achievement nobody has got legitimatly
