Headcrab zombies in Half-Life. If you can take something cheesy and overdone like zombies and twist it in a way that makes it scary again, you get awesome points. The headcrabs themselves were scary because you knew what would happen if they beat you (yeah, I know, not likely). I like all the creature designs in the old Half-Life because they were ALIEN, obviously weird, strange, not from this dimension. Very well personified. I still think it was a mistake to make the Vortigaunts bipeds with two grasping limbs, it's too familiar a shape.
Everything from Silent Hill. Great horror achieved through inscrutably twisted THINGS whose resemblance to humanity is merely vestigial. Did some strange force make these things? Were they humans who evolved/devolved? And what about the "normal" people you meet? Are they monsters? Also, great use of sound. Every critter came with a uniquely disturbing noise.
I know flying heads are overdone, but the first time you meet them in Doom 3 scared me pretty bad. Where is that crying woman? She says she wants my help? What's wrong with her, maybe I can do something for h---AAH! AAH! SHOOTITSHOOTITSHOOTIIIIIIIT!
Big Daddies and Little Sisters. Her with her little-girl looks and creepy good cheer, so out of place in the city of the strange, then she pulls out a syringe the size of a small dog and you're like ohgodohgodohgod. Him with his grim force-of-nature presence, so strange he makes the Splicers look downright normal.
