In Oblivion, where I was one of the unlucky saps who found themselves stuck as a vampire. During the quest to rid yourself of vampirism, the woman asks you to retrieve some one of a kind item and give it to her. I did it, and then afterwards she was still asking for it. So with no conceivable way to get it back there was no way to progress in the quest and no way to revert to being human.
The glitch wasn't patched until many months later; I had to start from scratch after sinking nearly a hundred hours into that one game.

Commando's 2 destroying all my auto saves (I was young, I thought they would last forever) and wiping dozens of hours of precise sigaret tossing and knifing germans from the face of my hard drive. During the final mission. I never did go and finish it after that
I have an issue wit Fallout 3 where it's RAM usage just continues to grow until it crashes. This means every 5-20 mins i have to restart the game, resign in to GFWL etc. It is so annoying I've never gotten around to finishing it.

Friday October 28th, Gemini, a new hero, was released in HoN. His ultimate is glitched so you can get 1.7k damage and 22000 hp, around 10x his normal stats. OH-KO's...
Halfway through BioShock 2, saves corrupted. Despite being really pissed off, manage to play all the way through it right after. I played it a second time, and the same thing happened at the same exact point in the game.
Ratchet & Clank: All 4 One has a few sections where the players have to lug around this baggage of crystals while recharging it with fragments along the way. Multiple times the carrier wouldn't spawn, so I had to restart the sections quite a few times.
Dark Souls, estus delay. For some reason my dude likes to wait a few precious moments to drink estus when fighting Ornstein and Smough. I can still move around, but right when I need to dodge my guy gets thirsty and refreshes himself with some estus while Smough's hammer has its way with my ass.
In Metro 2033, if you're crouching on a railcar during one scripted event you clip through the floor when it's done and come out in an endless void. The game then autosaves. You have to restart the mission. And even then, crouching seems like the obvious thing to do so you avoid smacking your head on a pipe, so you do it again next time, and next time, and next time until you make the connection.
Mostly I just laugh in disbelief and glitches, but there's one in Blur on a Brighton track in multiplayer where it transports you back to the beginning of the lap, so you can't possibly catch up to anyone.

Fable 2: The Bloodstone Banshee Glitch.
If you don't know, this glitch kills the fucking game. Seriously... if you get this glitch you need to start over. From the very beginning. Basically, there is a quest near the end of the game where you need to kill a Banshee on the docks of the town Bloodstone. Simple enough.
However, there is a glitch that means the Banshee takes no damage, no matter what you do to it. So it never dies. And as long as it remains alive it will attack you every time you go near it (which is often as the entrance to Bloodstone leads right to where it is) and you can't do anything else in that town until it is dead.
Oh, and that town is where the final quest is. So you cannot finish the game if you get hit with this glitch.
I'm sure there are work-arounds and I'm pretty sure it has been patched by now in updates, but when I first encountered it I almost just gave up on the game. I didn't though. Not even when I got it twice in a row.

when I was half-way through old world blues Just got the suit and the game crashed and my only save file for old world bues corrupted .

PS3 version of Oblivion. i got turned into a vampire which i wanted to cure, only problem is the quest is broken as the quest giver won't accept the items. only option was to buy the DLC with the cure fountain which there was no way in hell i was doing for something they should have fixed.
GAME wouldn't let me replace the game for a 360 version because they didn't believe my excuse. so it was a trade in for the game instead.

My guess would be something like "Talon" being a type of hero, and "ult" meaning his/her ultimate power, so "Talon's ultimate power", but that would just be a guess. I had gone to play Reaver's Wheel of Misfortune. Just bored and celebrating finished getting a certain group of collectables or something. I went in, pressed the button to spin the wheel. It didn't spin. I could not spin it again because the button prompt didn't come up, and worse sill the door had already locked behind me start the game.
I tried to leave via the quick travel and I can't because I'm in the middle of a mission.
I also somehow saved in there so no I'm stuck eternally.
I am still currently trapped in that play through. That was my best play through as well.

Oh and another I've experienced is that once I spent days on end building a massive castle and having mined thousands of blocks of stone for the walls.
One day I woke up and my computer had just eaten the save file. It was gone and nothing I could do could bring it back. I was furious with the game.
Batman Arkham Asylum
This was not a game breaking one in any way but was just quite annoying.
Recently playing through the challenge maps. I've already completed them all so not a problem. I got to the last room of the ordinary "Jungle Rumble"(where you fight a Titan and several henchmen) and started beating up everyone.
I decked every normal henchmen and was left with the titan but I could still hear a guy taunting me. I turned on Detective mode and saw one henchmen had somehow been launched through the window at the top of the room. I beat the titan but just couldn't get the guy down. He was on the other side of the window, so I could bat claw/ batarang/ sonic batarang him.
I had a good score going as well.
