Recently in Final Fantasy XIII. I'm sure it's in other JRPGs too.
Crushed to death by a giant turtle. And by crushed to death, I mean he took steps. In place. That hit my entire party for 1k each. With that kind of power, how is it possible other animals still exist in the wilderness here?!

Suddenly a Sage and a Hero come out of nowhere, the Sage takes away about half of my units HP with a really BS Critical and the Hero kills the Unit with a Brave Sword.
And this has happened more than once.

Dead Space has alot of the "because I said so" death, where if you don't see an emeny or if you're not carefull you get torn to shreds

I once saved a woman from wolves in Red Dead: Redemption, and right when she was thanking me, a mountain lion jumped out of nowhere and attacked and killed her. I killed the lion and went on my way, laughing about it.
Killed by tripping (walking) over a bucket while playing Gmod. Don't remember the exact map, but was playing a makeshift game of hide-and-go-seek with a few of my friends, and this bucket fucked me up.
I was playing spy in tf2 and was creeping up behind a friend of mine who was playing demo. I heard him sneeze on vent, which caused him to press the fire button. He fried a crit nade which bounced of the wall in front and landed on my head. Boom, splat and rage.

There's a multiplayer map in LP2 named The Great Outdoors which has a lake in it. Now this particular map is also in the story mode. So I got to this map in the story mode and thought I could flank the enemies by jumping in the lake as I do in multiplayer. But unfortunately for me the lake was filled with piranha akrid. Yeah, I didn't last more then 2 seconds.
Crysis was full of these. Doors going mad, shit just exploding, being blasted off the terrain only happened once. I suppose it only needed to happen once for the WTF to sink in.
Once in RDR, I was just galloping along in Free Roam headed towards Pike's Basin, and my horse just had a heart attack next to that huge cliff in Hennigan's Stead. The one overlooking the huge plains of Cholla Springs with Armadillo in the middle of it. Yeah, the HUGE one. I rolled off of it and died a quick, painful death.
