Halo. I was "killed by the Guardians" in the middle of a sniping spree (I wasn't moving around or anything).

Probably Bungie's way of preventing camping.

No, its a bug. They use Killed By The Guardians when the map itself kills you.

Yeah, it also happens when you walk outside the bounds of that snowy map with the giant gun turrets and they shoot you into goop.

This one time in the freeware game Spelunky the was an arrow trap ahead of me, so I threw the rock I had to set it off, the arrow trap went off, but the rock ricocheted of the wall and hit me. It was unexpected, so I just laughed. But seriously that happens all the time in that game.

Tripped over my skateboard in Skate 3 and died.
In crysis an enemy in front of me was shooting at my direction and hit the tree behind me several times. The tree broke and I got crushed by it.

Playing Jedi Knight 2. A dude is about to hurl a grenade at me. Since I'm an awesome jedi, I force push him before he throws it. However it seemed like he had actually managed to get it off, because the force from my push made the grenade bounce off the wall behind him and hit me in the face. Now I choke them instead, just to be safe. ^^

I found a hilarious bug in Sniper: Ghost Warrior (if you've never heard of it, that's for the best) you get a grappling hook and for some reason when I used it the game would fling my character into the rock wall next to me insta-killing me, this happened three times >.>
