Being knocked off a wall by sand bird-mosters (or bats for that matter) in PoP:Sands of time. It's just such a cheep shot really that's very difficult to avoid.
Looks like the Spider-Man developers have been reading too much One Piece to me...

Everytime in left 4 dead, when you gt hit by the tank etc etc, and end up landing from a height or something etc etc and insta dieing. Its very funny at times, but also annoying.

Well there was the one time where me and a friend were playing L4D2 and on the second safehouse in he jumps ontop of the table in the one corner next to the boxs, jumps down into the corner, then dies. Killed by a table, truly humilating.
There's also that incrediable complex trap set up in the one store near Tennpenny tower in Fallout 3. I managed to stop the dominoing boxes but then went and stepped on the floor plate they were going to fall on and set the whole thing off again. I didn't die, but it was just extremely weird.
And since I played Borderlands earlier today, I have to mention killing people with Sledge's Shotgun. That thing hits like a freight train and it always amuses me to see the enemy's body going flying after I kill it. Evertime I kill it puts a smile on my face.

Ugh, I hate it when that happens. And an enemy always seems to be near by for some reason.

Spawn killed in Modern Warfare 2, thrice... in a row! Something in my head kept screaming "How does a modern FPS ship with that bullcrap in effect?!?" Wouldn't it make sense to spawn somewhere AWAY from all the people trying to kill you??

For me and my friends:

Black Ops: I threw a grenade, it hit a tiny pipe and it bounced back and hit me between the eyes and detonated.

Red Dead Redemption/Undead Nightmare: 1: Ran a horse off the bridge to Mexico because I didn't notice it was broken. 2: (For a friend) Was on his horse in Tall Trees, a wretch-er that wasn't supposed to be there spat on the horse, the horse fell down the mountainside, survived, then was eaten by 10 common undead. 3: (For a different friend) Started killing wolves with a knife while hunting, there were about 10, two left, they Killed him. 3: (For a friend mission fail type death) A woman's carriage was stolen, he almost got away, after fiiiinally catching up and bringing it back the woman was attacked by a cougar while she was running up to meet with him. There are more I just can't remember.

TF2: Got killed by a Heavy's taunt three times in a row then the Heavy stole the intelligence and won the match. There were more but moving on.

L4D/L4D2: Accidentally jumped out a window in Dead Center. (For a friend) Startled a witch, closed the door and ran about halfway through the level (backwards) and the witch finally caught up and diced him. There are more but moving on.

I'll likely remember more soon.

Crysis - ridiculous physics collisions. You can walk on a pile of things that used to be a building (after blowing it up), and suddenly you die, because something moved a inch.

The Dead Space cutscenes come to mind, there are some ridiculously stupid deaths, which are also fun to watch :p

Get hit by an errant flame particle/flare in TF2 while you decimate the enemy team.

While standing over a pile of bodies, you are still on fire and at quite low health.
You call for a Medic, as he is right over there. Unfortunately, he's a potato. Glued to overhealing the same Heavy all match long. No Engineer has been brave enough to build/move a dispenser in the area. You spot your salvation, a medium health kit in the corner. You make your way over there, every second being vital due to the afterburn ticking your health down. You get closer, the health kit being less than a step away at this point. Your health is at three, but surely you'll get it in the nick of time.

You don't, and you die on top of the health kit.
Then the Scout at 120/125 HP grabs it immediately after if not right before.

Dark Souls, the fight against Artorias in the DLC.

First time fighting against him, trying to learn his moveset and figure out the correct ways to dodge them. I got most of the moves figured out, just had to dodge at the last second. He did his lunge attack that I barely dodged. I remember saying out loud "Got any other moves, because this isn't going anywhere?" He then proceeded to somersault, slamming his sword on top of me. "Okay, that's new" I managed to say while trying to get up from the ground. Artorias wasn't as impressed and somersaulted two more times before I even managed to stand up, killing me.

From that point on, it was personal

Half-life 2 and other games with Havok can occasionally kill you through physics failure. My favourite was one time in Half-Life 2, I was trying to drop down from a wooden plank about six feet off the ground, but I got a little stuck in a gap between two wooden planks. It was small enough to obstruct my fall but big enough that I sort of slooowly squeezed through, and when I finally fell through it, it rocketed me into the ground at a super-high speed and I died from that six foot fall.

Not technically 'deaths', but failed a mission in Far Cry 3 because I escaped 'incorrectly' from a platoon of mercenaries. After picking up the intel I needed, Jason uttered, ' I really need to get out of here.' So, logically I shot a guy, stole his jeep and fled...only to have MISSION FAILED!!! smack me int he face. Apparently this was not an acceptable escape. I needed to fling myself off edge of a cliff instead. Which obviously made much more sense.

Also, I seem to remember failing in Assassins' Creed 3 because I caught someone too quickly.

Crushed by falling wreckage in planetside 2. I'm running around at a techplant when suddenly a burning wreck on an ESF falls on me.
