Most "Oh, Shit!": When the "something just spotted you" music starts playing in Oblivion and I have no idea just what exactly found me. I'm searching around frantically, my pulse quickens, and if I hear one of the Beneath the Bloodworks vampires screech "I NEED MORE BLOOD!" I'm like "oh, SHIT!"

Most oh shit moment for me is in Fatal Frame 2, when you here all the weird groanings and incoherent mumblings of a ghost but you can't find it anywhere and you start pissing yourself in fear, since that fucker could come from anywhere.

Best: The reload noise for the crossbow in Half Life 2, cause you know someone else is about to get pinned to a wall, and there's nothing better than someone hanging off a wall from a bolt to the head.
Worst: The sound of an orb being released from an AR2 rifle in Half Life 2 Deathmatch, cause a bouncing ball of pure own is about to find your face.

Also I love the sound Ryu Hayabusa's Katana makes in Ninja Gaiden 2 as it decapitates a ninja.

I think the main thing is the timing...it doesn't start immediately, but a second or three after he starts to fall, and the pitch is just perfect.

Okay, here's a short list of my top sounds.

The Homeworld soundtrack (I know not really sound effects, but the soundtrack is THAT good).

The double barreled shotgun from Doom II. Every time I shot it I got all warm and fuzzy inside.

"Accuracy / Holy Shit!" from Quake III's Rocket Arena 3 modification. Awarded when you get a air rocket "Accuracy!" or air Grenade "Holy Shit!"

The gibing sound you get when you fall from a really high point in Half-Life 1.

The Ion cannon from Homeworld. It was all like "Bsseeewwwwwwwwwwwwwwwmmmmmmmmmmmmmmssshhht!" just do that in a deep bassy type voice.

That sound usually gives me a "Oh shit, there we go again." feeling, mostly because I hear it a lot.

My most satisfying noise is the sound a robot master makes after it explodes in the Mega Man games, and it's the most annoying noise when you're the one exploding.
