No recent monsters really scare me now that I'm older. Buuuuuuut, when I played Super Mario 64 I was young and that giant black eel monster in that cave world scared the crap out of me....

A lot of people mention the harder fallout 3 monsters, Mole Rats creep the shit out of me. Sure they die in like one hit, but they move quick and are pretty silent, and when they attack they make a loud noise. Just last night I was wandering around the waste and out of no where two jumped me, I nearly crapped myself. Also, ANY swamp folk, mostly the scrappers, scare the shit out of me. Really, all of point lookout was creepy.
*edit* Forgot to mention Dead Space, for me, I got tired of the method used to kill enemies, so the game eventually became less scary, but, it terrified the everliving hell out of my girlfriend, she couldn't sleep for days. Oddly enough, she was the one to make me play it.

AVP's Aliens. Not all the time though, half the time they're just there to kill, but the other half is bloody terrifying. Especially this one bit, you climb all the way up this building's stairways, only to have them come through this tunnel with little light really slowly. Motion tracker just beeps, you swear you saw something on the roof.. then suddenly, out of nowhere, a fuckload of them come at you. You HAVE to run, and when you're running down the stairs, the motion tracker going berserk, you can hear them jumping and clawing at you, screeching, I was on the edge of my seat literally until I finished that bit.
maybe no the scariest monster, but the scariest monster moment was RE2 when your in the interigation room with the one way mirror. I was playing at night time and there was no music (shoulda known then), after you pick up the item on the table a licker crashes through the window. that was the most freaked out i ever was by a game.
The crawling guys in condemned made me jump a lot, and that whole night my friends and I were imitating them and freaking each other out lol There were a few times things in Dead Space got me but those creatures arent too scary when you arent running out of ammo and running down dark corridors thinking "Cant turn around cant stop. What do I do??"
Protocol Droids in System Shock 2, when you can hear them but not see them.
Or from my very young days, The Demagorgon from Forbidden Forest, it's only visible when lightning strikes, it moves after each one, until it gets you.
And you have to shoot it between the eyes to kill it.

Dead rising's zombies once their eyes start to glow, something about that creeps me out.
As for bosses, I can't honestly think of a boss that scared me. The closest thing to scared I've been with a boss monster is intimidated, and that was easily the Nargacuga in MHFU, I thought to myself "judging by the way this thing looks it'll have Tigrex grade power and...what the hell?! Holy crap this bastard is quicker than a horse on pcp! I may very well be fucked on this on- OMG it gets faster when pissed and has a one shot tail move!? Yup, it's offical, I'm fucked." Course once I found out his attacks hit light and he's heavily weak to fire, he got raped roundly.
