Two words: Dead Space. I was hooked (and thoroughly disturbed) for most of the game. The first time I experienced one of those tentacle mini-games, I shat myself.
Plus the mid-game boss was a giant ass that shot poo at you... that was scary.

F.E.A.R. scared me to death, mostly because of a single incident. My little 6 year old sister, who bears a striking resemblance to Alma, once wandered into my room in the middle of the night while I was playing it, just as the freaky lights started happening. I don't think I've ever moved so fast in my life. Brrrrr.....

Hahahahah my missus deliberately did that to freak me out because I kept scaring her with pics of alma... waited outside the bathroom in the hall with her hair over her face... I nearly ruined my shorts...

Clock Tower. The ORIGINAL one that isn't supposed to appear outside of Japan....absolutely terrifying. When I emulate it, I play it in a tiny box with the sound right down. I've seen some very nasty horror films, my favourite genre, but in that game you just feel utterly helpless.

More recently I'd say Silent Hill 4: The Room. The room in the hospital with the giant head of your neighbour made me feel physically sick. Silent Hill 2 is on about the same level.

Project zero 2 [thats fatal frame 2 for you yanks :p]
not only was it scary with ghosts with plenty of 'jumpy' and 'oh shit!' moments, but it was about twins, and lets face it, twins are always creepy

oh did i mention the only weapon you have is a camera?

Either the Silent Hill games of F.E.A.R.

I play FEAR on my PC with headphones, and whenever someone is behind me and "taps" me to get my attention, I nearly wet myself.

But there aren't many things scarier than walking down a dark tunnel, and seeing Pyramid Head come from around the corner.

I don't know if mods count, but there was a Half-Life 1 mod called Afraid of Monsters. It was basically about a guy addicted to painkillers who starts hallucinating (or is he?) that he is in a nightmare world. Even with dated graphics, the monsters were creepy as hell, and the game frequently made you go "Oh Jesus no." The author used cheap tricks like turning out all lights in a level or creating invisible enemies you could only "see" with your flashlight. Playing it from start to finish, all after dark with the lights out, was an unforgettable experience.

S.T.A.L.K.E.R shadow of chernobyl... really no one mentions it as scary but it was so atmospheric and had the creepiest locales of any game.

F.E.A.R as well... it was almas appearances that made me go 'AHHHHHHHHHHHHHH!!!'

system shock 2 - scary as hell. even with its hideous graphics. i didn't find bioshock scary though, bar one of the first splicers (the one beside the baby pram with the gun in it)

admittedly i have never played a silent hill game, worth trying?

The first time playing resident evil (the first one), because it was kinda a first game I played of its type. but no other game has made me jump as much as that one (the first time you go into the corridor with the dogs, I litterally threw my control when I jumped, in fact my older brother jumped aswell)... Yeah newier games have come and gone but that one was the first horror type expeiace in a game I played so it just stands out!
