Getting locked inside a room that didn't exist til then in Fatal Frame 3, and hearing screaming and dead people on the walls for 3 minutes, without anything happening afterwards. The randomness is what makes it weird.
The room where you fight the first boss (the plane survivor) is creepy with lots of random events ocurring throughout the game.
The freaking shrine maidens, their rooms and the room where one is locked. And they're fucking ritual song.

Doom 3, several years ago. Game had some good honest scares beyond the jump-scares and the otherwise great atmosphere. Then I killed two Hell Knights with a BFG and was like "WHO'S A MAN AND A HALF! I'M A MAN AND A HALF!"
Then I went to Hell, lost all my weapons bar the shitty "plup plup" pistol that takes half a clip to kill a single zombie. Then the game threw another Hell Knight at me. I nearly shat myself as I panicked and put a good six clips in his ten-spot.
It was awesome, and genuinely horrifying.

Absolutely anything from Silent Hills 1, 2 and 3. Serious that stuff is just WRONG, then again that's why I love it so much.
Specifically the mirror room and anything involving Pyramid Head. Just seeing him the other side of those bars still makes me want to cry like a girl.
