The invisible water-monsters from Amnesia: The Dark Descent. I land in water and immediately start running when I hear those heavy "KA-SPLOOSH! KA-SPLOOSH! KA-SPLOOSH!" in the water. Never been so genuinly exhilerated to run away from anything in gaming ever.

The Regenerators from RE4. Powerful attacks, nearly invincible and utterly terrifying to look at.

When I first heard them breathe I started shitting my pants. The thing walked around a corner and suddenly stretched his arms out and grabbed me; started gnawing on my flesh.

Edit: Oh yeah this kind of regenerator is only seen once I think. The others are slightly darker, don't have spikes on them, and have a little less amount of leech things on them.

I found the Ichthyosaurs in Half-Life to be truly scary. I mean, they're pretty creepy to look at, but the main reason is that they're so dangerous. They move fast. They make that... sound (which I don't even know how to describe) when they're barrelling towards you to try to bite you in half. And you have to leave your comfort zone in order to fight them, because it's very difficult to kill the damn things from above, you have to get in there, in the murky water, where it has the advantage. Also, most of your weapons don't work underwater.

And then there was that part where you had to navigate the slippery, crumbling walkways above the pool occupied by two or three of them.

"Don't slip, don't slip"
*slip*
OH GOD
There's that one part after avoiding the chopper and swimming in the dam to that drain: the damn things get in your face! And I think it's sorta like a howler monkey's call echoing out of a tin can or something.

Also, Emerald Weapon from FF7: I never really liked underwater stuff to begin with, but I HATE the submarine section. His position is random, so he could be right behind you first dive! I've also heard he likes to hover around the crashed ship dungeon, which means he could be waiting to strike as soon as you leave. He's a hunter waiting to pounce!

The scariest enemy ever? Wolves from Penumbra: Overture. In most games, even the most terrifying enemy is just another bullet receptacle. In Penumbra, you can't do shit, and that was a truly scary feeling.
Lol, you can easily kill the wolves in Penumbra, you just stand on top of a box and wait for them to run up then you hit them without fear of dying because they can't touch you. At one point I wanted to rush through a level and I timed it right and waited for the wolf and then I beat the hell out of him with a pick-ax.

Although I can't think of any real terrifying one I guess I will have to say the water monster from Amnesia, you can't see it, you can't outrun it, it is loud as hell, it digests human limbs in record timing and you can't fight it at all.

And you run through that shit.

PS: I haven't played it but this looks pretty terrifying.

If you'd played it you realize how easy the game is and never be scared of it again xD

OT: I truthfully have never been scared of a videogame character before :p

From what I've played, either the Poison Headcrabs or the Stalker Necromorph from Dead Space 2

I cheated and turned up the contrast, then I proceeded to beat one to death with a barrel, but yea they are kind of creepy

The poison headcrabs from Half Life 2. I remember playing in one area of Ravenholm, I was up on top of a building, I saw a poison zombie down below, and before I could react, he threw one of those little bastards at me. Landing right in front of me. I found myself pausing the game and screaming in terror.

Looking back though, those things could be killed with a single swing from the crowbar. And yet back than, I wasted shotgun shells and grenades just so I wouldn't have to deal with them face to face. They were that terrifying.

Back when I was little, I also got freaked out by the Wolfos in the N64 Zelda games. Something about a wolf suddenly popping out of the ground howling didn't sit well for me back than for some reason.

I don't mean "scariest looking" or "jumped from a vent". I mean "eitherfightthisorsetselfonfire-fuckitwhere'sthegasoline". Enemies that make you sit on the edge of your seat because you don't want to encounter it.

Mine is the poison headcrab from Halflife 2. The prospect of losing nearly all of my health from one bite made me turn my speakers all the way up so I could hear the evil hissing.
The Regenerators from Resident Evil 4. One of my friends never got past them. Also, Bad Girl from No More Heroes. She kicked my ass so many times, I ultimately had to pull out the iTunes and the beer and settle in for about 2 hours. She had an instant kill and a few other moves that if you didn't react perfectly, you were very quickly doomed.

I'm not a big fan of swimming in open water, so "Thunder Ripple", the 7th Colossus, always terrified me.

Stalker: bloodsucker.

Dead space 2: regenerating necromorph

Half life 2: poison headcrabs and zombies that had them on their head.

Just to be different, I'm going to say The Dahaka from Prince of Persia: Warrior Within.

This badass dude will come crashing through a wall at any time and start hunting you down. The closer he gets the more the color gets washed out. If he gets close enough he will fucking absorb you. To get away you need to do death defying jumps across pits and running on walls etc.
YES! Oh my god, I live in fear of the Dahaka whenever I play Warrior Within. it's not just the running and death-defying acrobatics you need to do. it's that you do them WHILE he destroys the fucking area as he chases you, he can teleport, and if you don't do each consecutive move first time, you WILL die.

and he never stops, never rests. he will not stop hunting you, until you are DEAD!

Hehe yeah, but there is one good thing about him! His personal soundtrack is I Stand Alone instrumental version, :)
And some other instrumental songs from Godsmack.

Also: Nightkin in Fallout New Vegas. particularly the ones in the basement of the missile launch place (forgot the name).

You can barely see them, but you hear their footsteps constantly. Not to mention all the times you turn round and BOOM! Uncloaked one with a massive hammer!

Plus I was a Sniper! My only way of combatting them was to run backwards, but that inevitably led me into corners (where I was destroyed by the one chasing me) or into other cloaked ones.
