Majora's Mask creeps the FUCK out of me. An evil mask that possesses people and messes with everything it can, such as poisoning the swamp, snowing out the Goron mountain, and causing the fall of the creepiest moon EVER for the sheer lulz. And this is just Majora's Mask. What the hell is Majora himself/herself actually like in person? And just look at the damn thing sometime. Those eyes stare into my soul....

Anything that you cannot kil frightens me simply because you most times i end up running away not knowing how close to death i really am. The regenerating Necromorph in dead space is on example, that was the one scary thing about that game. And not knowing Where enemies are freaks me out also

The Librarian from Metro 2033. I would see one and think, "What are you? Please don't see me so I can study you. Is that a-oh crap run!" and would then promptly be destoryed by three of them.

Regenerators make my blood curdle. Dear lord that breathing...
Reapers from RE5 get a honorable mention for being scary, but more because they can kill you in a second.

The damned fish from Super Mario 64 that would just eat you outright... I still cant stand the thought of swimming in open water.

Man, those things eesh... play S.T.A.L.K.E.R.: Call of Pripyat and you get to sneak through a room of 10 or so of them sleeping :D It feels so safe.

I was in the habit of not even bothering to specifically avoid their bite when i played. Yes, it dropped you to 1 health, but that was only temporary and within moments it was already completely regenerated.

Only time you'd even have to think about the poison was if another enemy was just about to swat you, and usually there were no combine around, so you didn't even have to worry about bullets and stuff (see: actual danger for that situation). The headcrab zombies were slow and clumsy, so its not like they'd be getting you.
The only possible threat in that situation was a fast headcrab sneaking a hit on you right after the poison one did. A very rare case indeed.

As for enemies that match your description.
I can't really think of any, though i remember hearing that in diablo (yes, that old game) there was a zombie that when it hit you it permanently took away your health, which cased many people to quit their games and reset merely at the slight of them.

But that didn't apply to me, as when i actually played the game i didn't even know about what those zombies did, and usually was to busy filling them with arrows from a distance to be effected (even if they did hit me i probably wouldn't realise they took my health, being the young lad i was XD)

Similar to water in a sonic game. As soon as that music started playing you knew you were screwed, no matter how frantically you tried to find air XD

The damn Wasps from Silent Hill 3. Seriously...they grind along the walls screeching like metal-on-metal and spin like rusty tops. They're slow, but oh so hard to get away from when you're cornered.
I think I killed the first one I saw thinking it was a boss, then just ran like hell whenever I heard the others.

EDIT:
Alternatively, the invisible monster in the second level of Amnesia. You can hear it, you can see it's footsteps when it chases you and know it's big, but that's all. That thing makes me panic.

Bloodsuckers, anyone who has played any STALKER game knows how goddamn terrifying these things are. They like to lurk inside abandoned buildings, or out in the open, waiting for you to get near them before jumping out, slashing you once, then cloaking themselves to hide and wait for you to turn your back on them. At which point, they will wrap their mouth-tenticles around you head and drain all the blood from your neck. Your best bet for survival is to pull out the shotgun and run like hell.

And as if that wasn't bad enough try and spot the cloaked one in this image

That thing freaked the hell out of me when I first heard it! I killed one and said to hell with it, those bastards are ridiculous to kill.

The monsters from Amnesia: Dark Descent are freaking scary, and the regenerators from RE 4.

The scariest enemy ever? Wolves from Penumbra: Overture. In most games, even the most terrifying enemy is just another bullet receptacle. In Penumbra, you can't do shit, and that was a truly scary feeling.

The character I feared the most (at least as a young kid playing games at night in the dark) had to have been the T-103 (better known as Mr. X) in Resident Evil 2. He wasn't really anything special, but he'd come out of nowhere and all you could think to do was run.

The one enemy that has always terrified me and forever will; Puppets in Thief 3.


Possible Spoilers!!!

Once you get into the Cradle and find out it was once an Asylum it gets creepy, then all of a sudden, after 30mins of pissing your pants -nothing happens- then BOOM you see one in the corner of your eye... you sneak over and the lights start flickering.
You see it twitch as you sneak closer... and BAM! you shiv it with you knife, for it to only get back up after you turn around!!!

The only way to deal with them was loud, noisy flash bombs that pretty much shout out "look, I'm here, eat me!".

i was more scared of the tyrant in RE3. I saved all of my big bullets for the tyrant in RE2, but it was annoying the way he crashed through walls. The RE3 tyrant RUNS. And ALWAYS CHASES YOU THROUGH DOORS.
