Half Life - Maybe its just me, but everytime I get to the boss fight with that giant headcrab called Gonarth or something, he sits at the bottom of this cave and doesn't move when he needs to. It's a rare glitch, but I've gone through the game 4x up to that part and it happens EVERY FUCKING TIME.
Drop satchel charges on him. I kill Lord Bollock without even going down that pit.
I've actually had this happen a few times myself. I just revert to a previous save files and re-approach the boss.

you wanna know whats unfair?

NHL 09 - Be a Pro as a goalie on any difficulty above 'Pro'. (30+ save shutouts are a must if you want A win)
Why on earth would be a goalie on that game? So you can spend 20 minutes standing still and occaisionally letting in goals? I always play as a winger. Had one Be A Pro going with Savvy though. Oh, Savvy <3. Still bitter about losing in the playoffs to the bloody Hurricanes. Destroying the Canadiens 4-0 in their centenial year has to be my number one hockey memory though. Epic stuff.

Anyway, Prototype. You know that "mini" boss where you fight Elizabeth Greene in Times Square and she has all these awesome attacks? Well I spent two hours getting to her final form including dying. I saved it and has at her with a tank. The tank blows up. She does her desparation move where your health goes down to a slither but I was unable to run away after being ejected from the tank. Then a random infected slaps me upside the head and I die. Not to worry, I just saved it. Well, apparently you can't save in the middle of a boss fight so it goes back to the start. Still not completed it.

Unfair? IWBTG. Apples DO NOT FALL UP!

But beyond the obvious.

Earthbound, ready to take on Ness's Nightmare and I start the battle.

The shit face lobs a OHKO move in my face and sod's law ensures that Ness flops to the floor completely unconcious. This has happened to me more than once. Should not have given the Diamond Charm to Paula.

While it probably isn't the Most unfair thing I have every encountered in gaming, when I read the title the first thing that came to mind was Alpha from DOA4.

Space Quest IV.

You KNOW the part, but let me refresh your memory: Zero-grav swimming in the mall.

Being constantly stomped on by German pre-mades in my battlegroup in WoW. The view from the spawn point in Eye of the Storm is impressive.

Newest Wolfenstien final boss, damned annoying, and the mutiplayer for being good fun, then reseting my rank. Thanks Activison.

Gears 2 however takes the cake for having a car crash for mutiplayer.

Probably pretty much every boss fight in Metroid Fusion after the first 3. I w]ould fight it, lose, repeat this 10 times, then give up, then come back to the game like 2 weeks later. took me a very long time to beat.

Playing Dead Rising's notorious Infinite Mode and making it to day 4 just before my game freezes. I think a piece of me died that day.

There was that one time I lost nearly an hour of progress on mass effect because I forgot to save. Dear LORD I raged hard.
LOL, I've done that multiple times in Mass Effect. And like you, I raged, hard.

Another unfair moment for me is Total Recall and Darkman for the NES. Those games are full of unfair deaths because of shoddy programming.
