In TF2. I was ubercharged, got knocked into the air by...i don't know who out of the range of the medic, then was sniped in mid air.

For me it would have to be when i was playing halflife 2 episode 2 when i was trying to get little rocket man.(Bring the gnome from chapter 1 and put into the rocket). The hardest part of this was just simply touching the gnome. It acted like the gatling launcher from Left 4 dead. If i jumped on it I would go flying out of the map and die. When i was driving i made a hard left and had to put it back into the car i parked next to it and got out. The next thing i knew i was hundreds of feet in the air.

i'm not sure really, there was one a few days ago on COD:WAW where on the massive level with two big sets of steps to a large building on either side (i forget the name), anyway, I was inside one of the buildings in the middle crouched behind the blown out wall on the inside, reloading when someone on the opposing team came in, killed a team-mate and crouched in front of me facing the other way. A simple press of the right thumbstick was all it took heheh.
Also, playing GTA4 multiplayer once, I crashed a helicopter into the middle of the street where a big firefight was going on, but the blades were still going and they took out a couple of people, pretty nice to see.

In GTA 4 the missions with that reggae guy. I shot my partner a couple of times out of frustration but stopped before he died. Me and my brother were trying to squeeze every ounce of satisfaction out of his punishment, since we were both pissed about his stupid AI which led to us restarting the mission atleast 3 times. We then jokingly pushed him down a set of stairs (by walking into him), he fell and died...never thought that was possible but it was a good laugh before realizing we had to restart the mission again.
Getting pulled off by a smoker then to be jumped on by a hunter, right as a boomer vomits on me a getting swarmed by the undead at the same **** time.
Did I mention that it was one minute before the helicopter was supposed to arrive.

In any Battlefield: Bad Company, I just got shot by a shotgun, then healed myself. As soon as I had 89 health, I was sniped and died. Bogus.
Well it's not the most unfortunate but it is the funniest i've had on GTA, well i was in multiplayer tring to drive as fast as i could for no reason and, well ther's these little ramps on the big bridges and i hit one while driving into it at the side, fliped off the bridge and into and landed it 2 inches from a gas tank(on all four wheels might i add) (a very lonely gas tank that wasn't even near a gas station) and the car slides slightly into the gas tank and explodes, killing me and the person in the car with me. (the area was around where you kill.. someone who was troubling Roman. (trying to avoid spoilers))
the most unfortunate in my case would have to be when i died right as the plane was opening at the end of dead air.. on expert... which would have netted me, beating dead air, beating a campaign on expert (still don't have) akimbo assassin (i let my team do most of the work and i still don't have it), and no useing medkits (well someone told me pills don't count and i think dieing doesn't count towards it either and still don't have this one)

NPC: Walter in Megaton. His pathing got screwed up and I saw him fall down to the bottom of the city and die.
Me: I was playing Left 4 Dead alone and near the end of the Blood Harvest campaign. I entered a house near a cliff and had no idea were to go. I saw a window so I shot it open and thought I was supposed to jump out of it. So I jumped out the window and was rewarded with the image of Louis falling to his death.

me. Playing duke nukem 3d multiplayer I get blown into the air by a pipe bomb then blown up in mid air by a lucky RPG shot. Awesomely fail way to die. (So awesome I can remember the scene from nearly 10 years ago)
Right in the middle of pulling off Axl's Instant Kill in Guilty Gear X2...only to be killed by I-no's almost-no-damage almost-no-range crouching punch.

Fallout 3. I'd picked up the Experiment MIRV (or whatever it's called). Now, I realised it was a unique Fat Man (mini-nuke launcher), but I thought I'd get to a nice big hall and test my new toy out. Unfortunately for me, I didn't realise at the time that it fired a spread of 8 nukes in ALL directions in front of me.
Long story short, I aimed too low and the down-fired mini-nuke exploded almost at my feet >.<
I hadn't saved in about 2 hours either...
1.playing left for dead had 1 health about to get on chopper only to have a stupid zombie land in front of me knock me down and then have a smoker kill my friend.
2. having a laugh by locking safe room door on someone only to have them puked on screaming for their life and for me to open the safe room door. (tried to open it but there was about 50 zombie standing right outside of it killing my friend.

I was playing Halo 3 on local co-op with my brother and I threw a plasma grenade at a bunch of jackals and then he threw a plasma grenade and it stuck and my plasma grenade exploded and the jackal with his plasma grenade flew at us and the grenade exploded and we both died.
