Playing frontlines: fuel of war, on the single map with planes, and a friend and I where heading to kill someone in the back of our base. Whilst crossing the airstrip, someone started to take off, and since friendly fire was on, I decided to risk it and hope I got over. I got into his flight path, and panicing, decided to duck. I survived, but when I got over to my friend, he was laughing away. Turned out, my characters hat had been sent flying, and I spent the rest of the game running around without my hat.

Another was killing floor. It was the final wave, and I was warning another player about the patriarcs sniping skills with his rocket launcher. Cue the same friend again doing the ingame warning of lookout, and my shrapnel filled body flying past him. That had laughing for ages.

I was playing Fallout New Vegas for the first time and had just reached Nipton. For those that don't know, Nipton when you arrive is a town filled with heads on pikes, burning piles of corpses and anyone who was left alive was on a cross. I spoke to the man responsible (with help from others) from Caesar's Legion, telling me to go and speak of the horrors of the city to intimidate the Wasteland. The moment he stopped talking and him and his soldiers started marching out of town, what should come on my radio? Why, of course:

It's only the happiest rendition of "When the Saints Go Marching In" I've ever heard!

I couldn't stop laughing at some seriously unintentional black humour.

The entirety of Shadow the Hedgehog, especially the commander's flashback where he saw black doom. It just seems like a spoof, the kinda thing South Park would do

Dead Space 2, when you're trying to stick the needle in Isaac's eyeball...and you miss. For some reason instead of being horrified, I nearly wet myself laughing at the whole thing.

When you face Nicole in your mind at the end, if she manages to grab you at any point, you shoot yourself in the face with a Line Gun, I believe...also hilarious.

It probably says something about me that instead of finding these all weird and horrible, I laughed my ass off at them.

NOOO it cannot be stopped! Quick, kill it with fire!

My friend and I have found many gems in Oblivion from years of mucking about;
1) My friend's Khajit was caught in the act of murder and resisted arrest
Guard: This is part where you fall down and ble-
Ben (friend's character): HIARGH!
Guard: Blegh!
He cut him off mid sentence and the way it happened and the guard collapsed just made both of us burst out laughing.
2) Guard fighting mudcrab: "I've fought mudcrabs more fearsome than you!"
3) Poison apples (always seem to take effect at the most inopportune moment)
4) Reverse-pickpocketing clothes with a self-damage effect; on a mage, they chain heal, so their arm just stays permanently in the air till they run out of magicka and die.

Oh and there was that one time on Rainbow Six Vegas 2; the guards were shouting at each other on villa; we were in the garage, they were somewhere in the house so they sounded all muffled; me and my mate turned to each other and them simultaneously covered our mouths and mimicked them; to this day, whenever we're discussing RSV2, we have to have a laugh about that one moment.

Of course, I have to mention that time on Empire: Total war when the Dagestan declared war on me; I was the Russian Empire, on their doorstep, with an army three times the size of their entire military. Suffice to say the war was short, pointless, but did gain me some valuable territory.

I was staying up for a day straight to attempt to fix my sleep schedule, which was whacky as usual. Well, I was doing a round of Score Attack on Halo: Reach. Got in a ghost, and ramped through several grunts and nailed about four plasma grenades. They all exploded, I spun off - still alive - into the air and hit another Grunt. Fifteen charged plasma pistol bolts slammed into the side of my ghost, and then I exploded. My character's body shot out of the ghost, and wrapped around a pole in a fashion that reminded me of pole dancing.

As for moments of unintentional hilarity? There was the time I built to many units in Supreme Commander, and sent them to die against enemy defenses. They were mobile artillery and mobile shield units. Apparently this combination made the machines god-like and they carved a swath of destruction that basically drew out a phallus in the mass of enemy-colored tactical icons. I was laughing so hard I cried legitimate tears after the fact. My mom and sister separately asked me why I was crying and couldn't stop sobbing to explain to them the hilarity of my suicide troopers carving a giant hard-on into the enemy base.

EDIT: Playing Spore yielded great hilarity too. I had a fleet of war vehicles (was a civilization) and was all sorted for a conquest. Then I suddenly decided I wanted to make an economy civilization for sweet space age perks. So I switched over, and used my giant military to end conflicts. Sure, at the end of the scenario all the other city-states hated the Hell out of me, but when blimps and advertisements fly everywhere they suddenly forget that I pissed on all of them at the same time and buy my stuff. So I got the space age by being a capitalist asshole who shot everyone. And to make it even funnier the race of creatures were like fat velociraptors. My civilization were capitalist, fat, warmongering velociraptors wearing funny Napoleon Bonaparte hats and weird tribal feather clusters over their crotches.

Two funny moments from Exit:

Putting a patient on a stretcher and pushing the stretcher under a low ceiling with insufficient head room for the patient; the patient screams and falls off the stretcher.

Switching the direction of a conveyor belt with a child standing on it, so that the child falls off the end and screams when they hit the ground.

For me, I guess it would be getting carjacked myself in GTA. I couldn't believe that the game actually would screw me over like that.

I was doing my evil inFamous playthrough and I found it rather funny that the side mission givers always slowly backed away from me with a terrified look on their face as they briefed me on their mission.

Every time male Shepard tries to be suave. His voice actor is terrible and he looks like this:

Look at this face. Absurd.

I can't help but laugh.
This. And also Shepard dancing. I've tried "doing the Shepard", doesn't work very well.

Fallout 3. I go to some minefield for whatever reason, and notice I'm being shot at. I turn on vats and find out that it's an enemy sniper quite a bit from me in a building. But I ignore the distance, aim at his head and fire my Hunting Rifle. And what happends? I hit him right in the head, instantly killing him, making him fly back to the corner of the room he is in, flail about like a gary's mod character and then launch himself flailing about towards me and land just in front of my feet.

It was some mix of amazement and laughter.
