Fallout 3, the Mothership Zeta DLC. Slaughtering aliens in the robotics bay or whatever it's called, and one of them collapsed into a puddle. There was literally a puddle of alien on the ground. I hadn't even shot the damn thing, and I could still target it with VATS

inFamous 2: When cole finds a blast core and spreads his arms over it, saying "Ooooohhh, Blaaaaast cooooore!". I literally had to pause the game so I could crack up for a while minute.
Two moments in Bad Company 2 and one moment for my brother in Condemned 2. Me and a couple of mates were playing BC2 on the Heavy Metal map. Jumped into a tank randomly shot at the enemies general direction. Killed both my friends (They were on the enemy team) and the tank they were in. Also on the same mate with a few more friends that night i managed to park chopper on top of the apache in mid-air. Since that night i was never allowed to fly. 
And then there was that one time where my brother was playing Condemned 2, he was getting attacked by something, just as he killed it he threw the pipe in the wall. Said pipe bounced back and hurt himself....

In a old Game of Modern Warfare 2 I used Scavenger and threw smoke grenades Everywhere on the Rust Map. It might not seem like much but listening to the whining and complaining was funny to me.

Playing Smash Bros on the N64, I was using Link in the Hyrule Castle stage. I somehow managed to get my cousin, my brother, and a buddy, all trapped over on the right side of the screen. I'm sitting there mashing the A button until everyone is well into the red in damage percentage then UP+B for the kill.

I remember one time in DOTA, way back in the day, a player thrw Enchantress' (the Dryad model hero) Impetus (damage based on distance traveled) at a player who was teleporting away. The attack followed him back to base, where he teleported to the bottom tower, when it finally turned and hit him it did so much damage. Everyone in the game was Rofling.
One time when I was playing the first level of No Mercy in Left 4 Dead with a few friends, I decided to take the shortcut where you jump onto the side of the next building and then jump down to street level bypassing the first building. Anyway, none of my friends wanted to try it so they took the normal route and instead of waiting I took off and ran for the safe room. I managed to get up to the alarm car outside the saferoom and noticed my friends are still in the starting building, so to mess with them a bit I shot the car and ran and hid in the saferoom. I just stood there and laughed as they were slaughtered by the horde. And to put it in context of how silly this was, it was a normal game and they were good players.
Out of game moment would be getting drunk with some friends while playing Rock Band. The guy on guitar at one point hit the first note of the song, mumbled, "I can't see the colors." and then laid down on the ground for the rest of the song. Classic. I also apparently started swinging my drumsticks at the ceiling at one point.
In game would have to be playing Forza Motorsport 3. Was doing a sprint race against a Mustang with an El Camino and got super far ahead, so to be a giant dick, I braked and waited near the finish line for the other car. When they finally caught up, I hit them and spun both of us out. At this point my roommate started freaking out because I was backwards, he was facing straight and the finish line was like 20 feet away. So I throw it in reverse and cut him off and we drive through the finish, literally grill to grill. It was fantastic.

In Super Smash Bros. Brawl when I was playing with my cousin against two AIs. It was about 5 seconds before the end of the match and I was playing Lucario and my cousin, Ganondorf.
We were on a level with many different tiers and Lucario was standing a few paces behind Ganondorf on a platform. Meta Knight had just jumped up and barely, by a hair, missed an attack by Ganondorf. The funny part was I had charged up a massive energy ball with Lucario as Lucario was on 300% damage. The result was a perfectly timed (and by that I mean a complete awesome accident) Meta Knight performing leap frog before getting nailed in the face by a boulder of blue and being blasted off the screen in a flash of light.
The hilarious part however was two seconds from the end when Peach had jumped up onto the platform as well and my cousin had been pressing the kick button to hit Meta Knight before I had blasted him off the platform.
I'll remember the moment forever. As some people know when a match ends in the game everything goes slow motion before it cuts to the score screen. And in those two seconds of slow motion we watched Ganondorf - and my cousin has dubbed his kick the 'Boot of Doom' because it really crushed into someone's face - crunch his boot into Peach's face in slow motion... full with the heavy kick sound effect in a complete vacuum of silence.
Priceless.
Oh and also, whenever Snorlax comes out of the Pokeball. I laugh everytime.
