Not even this will protect you from a 50 cal. Only MW2 decided that it would. But put a 50 cal. towards any part of your body and your still dead.

Anyway, I say Mass Effect. Again, all the aliens look exactly like humans with weird faces. What are the freakin' odds of that?! It's still fun though. Just not realistic.

I think Insurgency covers this. You don't die you come back as 'Reinforcements' The game ends when all the Reinforcements are expended. Something I never understood though... On maps for Joint Operations, people bitched that it was unrealistic for the AI to respawn, but they were perfectly find with them shooting you through bushes, mountains, from a mile away, through fog, or a combination of all those with perfect accuracy.
And, pretty much every online shooter game is unrealistic. Why? You come back to life when you die.
