Mirrors Edge. Just all of it. The game isn't colourful, it is just one bright block of colour after another. I can remember running through a corridor where the walls were just one shade of bright red, so much so that it made my eyes hurt.

The level Cortana on Halo 3 also sucked. The main issue was that everything looked the same, which is fine aesthetically considering that the flood have taken over High Charity and completely done a number on it, covering it in biomass and turning it into one space faring hive, but damn, I couldn't tell one corridor from another.

The fade in Dragon Age: Origins was just awful, good thing that this mod exists for the PC version of the game. http://www.nexusmods.com/dragonage/mods/816/?

One game i played that made my head ache was a flying game called Descent. You flew through a mine and could fly upside down or on walls. Was really confusing when i played it because everything looked the same.

Playing as an alien in any AvP game. If you can't run at breakneck speed over the walls or ceilings of a corridor then you'll be dead at the first sentry gun or marine you see. And at those speeds you can't tell which way is up or down half the time. And some hardware pushing games for the ps3. The frame rate can be so irregular that my eyes get tired.
