Eternal Sunshine of the Spotless Mind. A truly brilliant romance that messes with non-linear time and light sci-fi concepts to deliver one of those experiences that clicks as you watch it, but you won't fully comprehend the entire thing until you watch it again.

Inland Empire is the biggest mindfuck ever, even moreso than any of Lynch's other films.

Enter the Void is also a massive mindscrew but it's more the trippy imagery and the way it's done the actual plot (well as close as it has to a plot) isn't all that hard to figure out.

Blood of a Poet is amazingly weirs, strange ideas and bizarre imagery, it's quite old so I don't know how easy it would be to watch it but its very good.

Although I'd avoid watching these until after some of the ones people have already mentioned because they really desensatise you to mindscrews and they're vastly weirder than alot of the films people aqre mentioning.

I say Videodrome. I've been renting backlogged David Cronenberg movies for awhile, and I would recommend Scanners, but there is almost little to no thought, but a movie that messes with your head would be Videodrome.

I would have also recommended Hausu, but that's an experience, not a real movie and it's less thought heavy.

Brazil is a good one too. Actually, the oeuvre of Terry Gilliam should count.

the movie makes perfect sense.

but when you find out what's going on,

You'll be lost at the beginning,

You should watch memento.

Pan's Labyrinth is a good one (also my favorite film). As a lot of people have said, Eternal Sunshine of the Spotless Mind will screw with you. Mulholland Drive...

This is very hard not being able to look at my movie collection. Damn being at work.

EDIT: I remember one! Even Dwarves Started Small.

Seriously... The fuck is going on in that thing...

Satoshi Kon, doing the Inception thing before it was cool. Rest in peace, you grand film visionary. His Paranoia Agent also comes to mind, but that was a series and not a movie.

Anime like the aforementioned mostly come to mind for me. Evangelion, Big O, etc.
