If Dark Souls is considered Medieval then i would like to point out Dragon`s Dogma.

Dark Souls isn't Medieval, it's Fantasy. You'll find in 6th-15th century Europe, there was no magic swords, dragons, or monsters.

So are you looking for -fantasy- or -medieval-? I'm going to assume Fantasy. Elder Scrolls Morrowind, Oblivion, and Skyrim. Jade Empire. Thief: Deadly Shadows.
