I would also like to toss in my vote for Kingdom Hearts 2. It continued the storyline well, and they added quite a bit to the gameplay.

Discworld definitely has gotten better and better as it's gone along. I agree that Storm of Swords is by far the best in the series, but A Game of Thrones is better than Clash of Kings. Other than a few key parts, I felt Clash of Kings drug its feet a bit. Plus, A Game of Thrones had probably the most memorable, game-changing event in any book I've read (no spoilers here, but if you've read the series I'm sure you know what I'm referring to.)
