Since Batman's love intrest was killed off in the last movie, I would expect a female villain to enter, probably Catwomen if any.

Harley Quinn becomes a Joker supporter while tending to him in the asylum he is inevitably sent to. The original Joker soon dies in some way or is shown to have died (since no one could pull off that act properly), but other classical incarnations of him appear as various crime leaders trying to take control of the city in his name, as though he has become the center of a new cult. Joker-worship unites gangs which previously fought one another and a powerful new crime wave sweeps the city.

I'm hoping for the internet rumors to be true and either Johnny Depp or Hugh Laurie as the Riddler and Philip Seymour Hoffman as the new Penguin. And imho i think Johnny Depp as the Riddler could be the only way possible to get the character anywhere near as good as Heath Ledger's Joker.
