I can answer your second complaint actually, since it's highly implied and hinted at that in the Contact Experiment with Adam when they found him in Antarctica that they tried to inject Human DNA into him (for some reason) and tried to apply restraints to him. He obviously went berserk and so they stopped him using the Lance of Longinus but not before he caused Second Impact.

After this, what was left and found was most likely the Adam Embryo (that Gendo obtains and places into his hand) and Kaworu with Adam's soul. He's basically Rei but Adam version, this is also shown as the Mass Production Eva's in End of Evangelion have Dummy Plug systems with Kaworu's name on them and so therefore powered by Kaworu clones, just like how NERV's Dummy Plug's are powered by Rei clones.

I'm going to say Diablo's universe. Honestly how is anyone still alive? You have massive 100 foot tall demons constantly laying waste to all around them. If you not a PC, the best you got is a few guys with a bow and arrow.

40k, specifically the Horus Heresy series. The Emperor acts like a giant twat to pretty much everyone and deliberately plays his sons off against one another and plays favourites...he then is left confused when the galaxy errupts into civil war as the neglected sons try to get some attention from daddy.

I remind you that The Emperor is supposed to be like a diety with strong powers of foresight, incredible intelligence and planning skills.

I do not like the crazy mix of 1300-ish technology with current technology. They use candles to light their houses, but yet Twilight has some sort of advanced brain scanner in her library?

If someone is tossing their cookies they always do it in a trash can, but never a toilet.

EDIT: This really isn't a nitpick, but just something I find odd. If I really wanted to nitpick I'd say if the major criminal organizations like Team Rocket are so well know and evil, then how come no police force goes after them all them time.

The only nitpick I have for One Piece is the lack of female priates.
No I'm not talking about Luffy crew or any secondary characters crews like Whitebeard of Shanks, I mean when it come to background pirate crews. Ok I can assume there were hardly any females pirates in a pirate crew in real life but come on, add a few more! It seen abit sexist for excluding them when it come to drawing some cannonfodder/ background pirates.

I know, it's awesome being bilingual, but you don't have to rub it in our faces! And why didn't they throw the ring into the fires of Mt. Doom when Sauron was defeated the first time around? That would have really saved us some time! But no, make some stupid Hobbit kid do all the work for you! Do you know how many people died because of that?

All the planets must just happen to be the same size as earth because the characters are not seen either bouncing up and down/wearing heavy boots or dragging their feet along.

Also the Marvel universe:

All of their superheros live in the same city, but whenever someone tries to attract a particular superhero by attacking random people, the superhero that turns up always just happens to be the one they want.

I think that's really subjective. I always thought Zuko and Katara were very interesting characters.
