I had a very odd moment in Fallout 3 on 360 just today. I finished putting the satellite dish thing on the Washington Monument, and didn't feel like taking the elevator back down. So I pull up fast travel just to see if it'll let me do it, and it does. With excitement, I go see the radio guy and he tells me to go to Rivet City, and gives you very good instructions on how to get to Rivet City.

Unfortunately, giving me directions is like giving glasses to a blind man, so I wander around for a while. I see the Washington monument, and wonder if maybe there's a tunnel underneath it (due to a faulty self-made waypoint). I get on the elevator and to my great surprise, it GOES DOWN (not supposed to do this, in case you don't know). At this point, I'm like "woo! I'm going underground!" I'm watching the elevator's descent...and it is plunging down through solid earth! I find this odd, but no matter...just a minor glitch in the graphics...sure there's a hole somewhere I'm going down. After descending for seriously five minutes...I'm getting upset. "What a ploy to lengthen gameplay...a five minute elevator ride. What bullshit!" I think to myself. All of a sudden, the screen flashes bright red, and I hear the sound of the doors opening and the door opens into a solid white room. I walk out, and my game freezes.

I think it happened because I fast traveled with the elevator still recorded as being on the upper floor, but I'm too lazy to confirm.
That kind of happened to me, I had fast traveled from the monument and then went back to ride the elevator. The thing goes up, no problem, the door slides open to reveal... the ground floor, albeit with some weird graphical additions like a lack of rubble and a pedestal that wasn't there before.

I was playing WoW and I used sprint to get to the elevator because it was about to close. I got to the door and it slammed down on my character shooting me through the floor and killing me.

you have to rescue your kid from the cave.**

I kill everything inside it and when i come out he's standing with his face against the wall like in the Blair Witch Project. He wasn't responsive to any of my emotes and my wife who for some reason followed my through the cave didn't know she was my wife and just started walking around the area.

This is an old one from the original Duke Nukem game.
I got killed right at the end of a level, but managed to get through the exit to the save room before the death animation completed. As soon as I appeared in the room the death animation started again and I died. I respawned at the last save room which happened to be the one I was standing in, with the amount of health I had when I entered it, which happened to be zero, so the game started the death animation again and killed me, causing me to respawn again in a never ending cycle. I had to load a saved game to get out of the glitch, but it was certainly interesting.

CoD3, right after you mortar a Nazi road block, you're supposed to go up to a friend and get a boost out of the foxhole. I thought I could jump out, and I did. I then fell through the map and finished the level by shooting enemies from underground.

Well, this one time I was playing Midtown Madness 2. I was in the city that's not London (can't remember what city it was) and I hit the ground hard after doing a particularly large jump off a hill. The weird thing was that as soon as I hit the ground, my car bounced straight up about a mile into the air; we're talking like ninety degrees here. It was bizarre.

Also, I remember one odd event that happened to me and a friend when we were playing Smash Bros. Melee. We got into the character select screen but the announcer guy started spouting out all the things he says in the game right then and there. There didn't seem to be any particular order to it and his voice had this weird grainy quality. Also pretty strange.

At a LAN-Party we were playing Flatout 2. We had a destruction derby going on with all 8 players riding busses. One of us gets then stuck to a corner and as he's trying to turn the bus around others start to notice that he's stuck there. So naturally we start to head towards the guy who's stuck, I think there was 5 busses heading towards him.

We crash into him about the same time. Next thing we know there's 6 busses inside each other in that same corner. At that point the physics engine desided to raise it's hands and give up and the game ended up chrashing on all of us. It was one of the most hilarious things I've experienced.

About 5 minutes ago, on Crackdown, driving the Agency Supercar. Some kind of glitch occured when I ran into a police car, because I was launched 400 ft in the air, bounced off the Central Point building, spun through the air between two of the agency towers, and landed on the Los Muertos Island. It was AWESOME!!

Also, not exactly a glitch, but driving the warthog onto the Scarab in Halo 2 on New Mombossa. Has some bizarre AI reactions. Apparantly it was even expected, as one of the marines in the passenger seat shouted "What the bloody hell we doing up here?"

I can remember back in the good old SNES days a basketball game that my mate had. I think it was NBA Jam or something like that. Anyway, we were playing this game when he went to go for a dunk, but somehow he missed it and hit the rim and his player and the ball shot into the air for a good 20 - 30 seconds.

Thee only really memorable glitch I have was from my adolesecencehood in gaming, where I was playing Super Mario 64, and playing the first level, the game glitched and Mario started to jump rocket high. Every time I made him jump, he jumped so high he even touch the sky level of the map. At first I reckoned "pretty sweet, this'll get me anywhere fast!" but after the first fall where Mario's overweight made him lose close to 3/4 of his health, I started fearing I wouldn't live long if I had to stomp goombas. At any rate, I died, and the game froze, so I decided to take the game out and put in Mario Kart 64, which for some reason, was also glitched, in the way that, every time I went to grind, the kart just leapt of the ground several meters instead of the usual tiny bounce, which made me lose several races, and I haven't touched my Nintendo 64 ever after that, in fear that the Pokemons in Pokemon Stadium started sommersaulting for no apparent reason.

A couple times in The Force Unleashed, some enemies had their body parts slip under the game map. An other time, I launched a stromtrooper into the air, the camera focused on the Stormtrooper and all of the sudden, there was a bright flash for no apparent reason. It made him look like he was going out, Team Rocket style. And the final glitch for today, I had a Stormtrooper pulled by an unseen force into the groyund, up to his helmet. While this was happening, he made no nosie and did not move.

L4D, the death toll campaign, combined with the minigun exploit (the one where you melee it, the stand on it, and it propels you) - we fired ourselves onto the roof of the last spawn point before the radio building. Then one of us set off the horde, and legged it back onto the roof. We all had hunting rifles, and expected to have a nice while of sniping and trying to avoid smokers and hunters. What we got, was nothing.
Just nothing, common infected, no specials, not even any tanks, as if they all died the instant they spawned, and a few minutes later, the boat came to pick us up. I should explain, that roof is by no means a safe haven: one time when i tried to recreate it, the other 3 stayed down and a smoker just pulled me straight off. Sever WTF moment for us all

The best glitches I've ever had have been in Bethesda titles. Oblivion is a common culprit and never before have I laughed so hard as with these glitches/bugs:

I was working for the Dark Brotherhood and I had to do the mission where you sneak on the pirate ship and kill the captain. I didn't bother going in through the ship itself or the crate you and stowaway in, I had high Acrobatics and Lockpicking at the time, so I jumped on to the balcony at the back of the ship and picked the lock. I sneak in and find the captain eating, so I draw my weapon and sneak-attack-Critical-Hit him for 2/3 of his health!

I get the "NPC wants to talk" zoom-in thing and my screen is full of the back of his chair. I can only see the side of his face as he looks over his shoulder at me and says something like "Yarr, you ain't one of me mateys!" and then I exit the conversation zoom-in. At this point I'm laughing so hard I can't work the keyboard to fight him, but luckily he doesn't do a lot of damage to me. I clock him in the skull with my sword once to finish him off and leisurely stroll out the door I came in through.

The funny part for me was the "Ya think?" moment I had when I slashed his spine open and all he could say was "Yarr, you ain't one of me mateys!" (paraphrasing, again).

Did you know that if you enchant a weapon to do 1 point of Fire damage for 1 second over 100 feet you can air combo stuff?

Learn some kind of spell that paralyzes a target and enchant a weapon in the way I just described. Paralyze something (like a bear or town guard) and hit them with your enchanted weapon. Watch them fly. Get under them and keep swinging. Watch them enter lower Earth orbit. Watch as paralysis wears off and they flail maniacally as they fall to the ground. Die laughing.

Bribe every single guard in town so they like you at 100 Respect/Reputation/I-Like-You-This-Much-Points. Bribe to 100 your victim, whom must be someone that will fight back. Punch your victim while no one is looking so they chase you. Run up to a guard and let your victim hit you. Yield to your victim to he leaves you alone. Laugh maniacally as the town guards swarm and slay your victim for you. :)

I did this to most of the guild members in Bruma (both Fighters and Mages), Maglir and Oren, and the captain of the Bruma town guard. XD Yes, the town guards killed their employer.

I know I described how to do most of these, but I guess what makes them "once in a lifetime" are all the specific things that resulted. Sometimes my victims reacted in less amusing ways and sometimes they did things completely unexpected (i.e. I did the paralysis-fire-weapon trick to a bear and I swear he flew so high he despawned). The town guard trick was pretty flimsy, especially.
