While playing left 4 dead i got a combination of the invincible zombie glitch and bad witch placement. First the withch was IN THE SAFE-HOUSE and she had the invincible zombie glitch, it was on expert too :*(

I was shooting at a Deathclaw in Fallout 3 from a distance, and it flew 500 feet into the air. Only for it to come back down near some Brotherhood of Steel outcasts and nearly kill Dogmeat. (Stupid suicidal dog.)
Tons of great glitches on the PS1, though.
Fun thing to do is take the disc out while you're playing. I did it with a Spyro game once and the ground became the sky and only the rocks and buildings showed up. It was fun, though. I was able to play for a while. :3

Ragdoll physics are always good for a few moments like this.
One time in Oblivion I killed a bandit with a nice backhand power attack, she flies up in the air and lands sitting on a barrel against a post as though she was placed there. Another time, I killed a Goblin with a fireball and its hand carrying a club got wedged in the rafters in the ceiling of this mind, so now I had a dangling dead goblin pinata.
I seem to recall mounds of defeated Longbow blondes [http://lh5.ggpht.com/_i-hHb1N868M/SCuQi8E6y1I/AAAAAAAAAu8/dWIa2kbVW8E/longbow_salute1.jpg] in City of Villains falling into promiscuous poses - copping a feel or going for a dry 69. Purely a random thing that came about from sheer number of them you're taking down with powers that knock them around sometimes, but good for a giggle nonetheless.
Then there's Data Realms' Cortex Command [http://www.datarealms.com/]. It's one long procession of incredible glitches because of the physics involved.
That's just par for the course.

I was playing Call Of Duty: World at War (and I was very skeptical of the game) on the very first or second level where you drop the nuke on the island. The very first time I was jumped by a crazy person with a bayonet he shoved me into the wall, after deflecting the attack I got up and couldn't move, then realizing what had happened, I had to restart the whole level over and couldn't believe that the first time I played this game, something like that would happen. Has that happened to anyone else?
Most of mine have been Halo 3 glitches.
Playing the level The Covenant, I had about three or four glitches in one game.

First was fighting the second Scarab. We'd killed the first with ease, the four of us together, and now we decided to attack the second. Suddenly, a Banshee from the battle overhead swoops down and I board it. Only, I had no shields, and was quickly gunned down.

Second, was the destruction of the second Scarab itself. As it lowered, our Marine buddies were trapped underneath the main body, in a Warthog, getting pinned. As the Scarab exploded, two of us died in the explosion. But, as the debris collapsed, we saw two Marines walk out from the middle, and survived the explosion.

Third, we had one person stand atop the Elite Phantom, as the rest of us died. As we spawned on it, we attacked each other. Only, the bodies didn't stay on the Phantom; they fell through.

Fourth, we kept the Scorpion alive, intending to use it in the Citadel. We accidentally drove it over the part where the bridge activates, and the bridge activated itself, killing everyone in the tank, spearing it and destroying it.

I was playing Assassins Creed. I was just walking around, minding my own business, when suddenly, my character was launched into the air, as if he'd stepped on an invisible land mine. I caught a glimpse of the clouds, but soon he fell and died.
Ass. Creed. Guards humping barrels and walls. too scary. The noise was scarring as well.

In Fable 2 I sacrificed my wife at the temple of shadows (to get that one special sword) and she got turned into a guy instead of dying so then I had a husband (I'm pretty sure that was a glitch in that she... he should have divorced me) I didn't really like that too much so I tried to kill him a little while later to start over, after killing him I came back and he still there (interacting as normal, very upset at me but refusing to divorcee me... I think I married an Irish) and he can't be hurt.

Fallout 3...I dont even know how this happened...I was in an intense firefight, then whipped out the Fat Man MIRV and fired.
I was breifly aware of there being an almighty explosion in front of my character before he flew into the skybox in 6 different directions. Either my Fat Man had malfunctioned and exploded as i fired, or someone was an INCREDIBLY good shot and shot one of my mini nukes just as i fired.

I fast travelled to the imperial city market district to find myself outside of the city, though I could see inside it as the CITY WALL WAS GONE. It was still there as i couldn't wa\lk through it... but it was totally invisible. Apart from random floating polygons.
There is a part in fallout 3 where some dead raiders are floating in the air above you. They fall if you shoot them.

I'm not sure if anyone has heard of the Yao Guai glitch in Fallout 3? Basically, using a Paralyzing Palm on them when they're in mid-air sends them rocketing foward in the direction they pounced.
Well, I tried it out, and as it flew past, it hit a Protecton square in the face, causing it to fall off the cliff we were on. I laughed so hard, I didn't notice the Deathclaw behind me.

In Stalker, even when I'm not bleeding, sleeping for more then an hour kills me. And sleeping for one hour puts me in critical condition. Unfortunately, that means if I want to wat out the night, I pretty much just have to screw around for 8 hours.
Oh wait, how could I forget the time when i, somehow, cart-tilted a freaking Wii game? I was playing Twilight Princess, and all of a sudden as i took my iron boots off, Link started seizuring as he was walking about. Something tells me it may have been because i was using the Fast Iron Boot Walk glitch...
Weirdest one for me was on Gears of War 2, on campaign with a friend, my friend's net lagged out for a second and at that moment I ran out of cover to chainsaw a Theron guard and the animation went through but the theron guard walked around as normal while the another locust about two feet away from my friend exploded in a mist of blood.

Playing on hard was a total *****, I made it to the second form of that spider-ant-wtf-thing, okay, so I get killed like 50 times, on hard mode, and when I finally had her on low heath, me and her both had low heath, and I pulled off an epic slash thru the other guy and see who lives move that you rarely see in actual gameplay.
...Charged the sword and went for the kill, she also went for me at the same time it was epic.

...I saw a flinch in Ryu before the screen went black signeling that he might have survived or died.
...The Black Screen Fades After A Minute...

Playing Halo 3 Multiplayer. I threw a frag grenade, and it seemed to bounce off the wall. My friend who was moving along the wall to check the doorway got hit by this frag rebound, somehow, it got stuck inside his character model.
A little bit of smoke was trailing off him and then he died. I managed to sticky a Frag Grenade. To this day we still wonder.
