I Love the fade.
My single favorite segment of either game, Dragon Age 1 or 2, hands down.

What isn't to like? Solo challenge, unique abilities to play with, fancy (though admittedly uninspired) puzzles, free stat boosts, really great aesthetic.
I have two problems with the fade.
1: Not enough of it.
2: They never truly got into the puzzle mind game spirit and went all out with really difficult mental challenges.
...The Fade has a great aesthetic? It's all mundane red and brown. I hate it mostly because of the way it looks. I was so happy they overhauled its look in Awakening.

Any location in games with random encounters where there are no safe tiles (caves, dungeons, factories, hostile castles, etc etc.) It's not that they're hard; they're just REALLY tedious. I don't like taking five minutes to move twenty feet. It's even worse if there are little traps and such that make you have to DO IT AGAIN. Heaven forbid it's a maze.. I'm looking at you, Relic Castle!

I'm strangely ok with world maps with random encounters, though.

Forest, underwater and cave levels tend to suck. I've played very few games that have interesting forest levels. Alan Wake is an example exception; although it can get samey. Who thought going underwater as Sonic the Hedgehog was good idea? No one makes interesting caves. Have you seen Rango? That's an interesting cave. At least try to add something distinguishing to your caves, devs.

I dislike the abandoned,and corroded vaults in both of the fallout games, particularly Vault 34 in Fallout:New Vegas. The whole place is dark, and the entire place is irradiated, and it is populated by ghouls that took way too many bullets to kill.

This is exactly why I hate the Fade: it's such a missed opportunity.

And that is why I do that last. If you aren't a warrior, melee rogue, or Arcane Warrior Mage, consider yourself screwed. It's just so bloody tedious! I think Bioware knew what they were making would be nearly as hated as the Water Temple in Legend of Zelda, so they threw in permanent stat boosts to make you want to explore it.

at least the industrial districts can be open, sewer levels are required to be in small confined spaces that are also usually dark
I don't really mind Sewer levels to be honest, it's the subway levels I really hate. At least in a sewer there's no place for the enemies to hide.

In oblivion the shivering isles expansion when you are on the mission for the chalice of reversal or whatever its called. That mission is horrible.

Yeah that one pretty much sucks. There was always a section where I couldn't find a bug with any of the freakin' felldew so I went into stupid major hardcore withdrawal.

And I think I'll add another: Killer Croc's lair in Arkham Asylum. That place just sucks. It's not a challenge, and it takes forever. It's just dumb.

Motels rooms with lots of identical looking doors.

Example: Remember Bluewater creek motel from Silent Hill 2, that was part horror and part
'Why the fuck has it taken me 4 hours to do this one level'

Seeing the amount of hatred for dark dungeons and sewers (I loathe them too in any game) makes me question why they seem to be in every RPG ever.

I'm going to have to parrot the DA:O Fade with everyone else. It was fun the first time learning the shapechanging, and I didn't find it difficult, but it just dragged, and I found the whole mages circle + fade bit really offputting for future playthorughs. Every time I thought about going back to that headache-inducing vaseline-smeared grey view I just can't be arsed.

Also when replaying a game, any section where I know there's going to be a timed mission or an escort mission, I'm likely to drop it.
