A fail moment for me would be in Left 4 Dead when it first came out, a Hunter jumped on me, started tearing me apart, my mate got it off me but I was still on the ground with blood coming out and it wouldn't kill me or stop. Had to quit the game.

Anyway, for another fail, I was playing a game of Insurgency[footnote]A must-have multiplayer Source mod, it's very much like Counter-Strike.[/footnote] and a round begins. So I was a Insurgent (terrorist) and spawned (obviously) with around ten other guys. Some guy who is an RPGer accidentally hits the fire button and teamkills us all.
I did the math: There was around 11 of us, so that means the guy was probably kicked as you're only allowed five teamkills in the server I was in.
Second of all, one teamkill means losing a whole twenty five points. If he killed eleven of us, that means he lost a whopping 275 points.
Building on from point #2, you would need anything from 138 to 34 kills before he regained all the TK points he lost.
And, because of him the Marines (other team) had already began overunning our defenseless base when I spawned.
So, in other words, shit hit the fan big time.
