Collar bone(soccer accident), ankle(soccer accident), a few ribs(car accident), leg(biking accident), fingers and toes(numerous times), and a fractured skull(car accident).
I've played sports for most of my life (I'm 23) and I'm as clumsy and accident as they come. I broke my wrist falling off the top of a slide when I was five, I broke my ankle by getting hit by a car, and I've had multiple broken fingers and toes. No lost mobility though, which is nice.
I have dislocated my arm, though. Can't remember the incident myself, 'cause I was just a little kid, but my parents told me I was in a lot of pain, apparently.
I broke all the bones in my left arm. They all broke in the same accident, so I don't know if that counts as once or not. That's the only time I've broken any bones.
The closest I've come to breaking a bone was my thumb. During a game of basketball the tendons and muscle got ripped and my thumb bent over in a direction it was not designed to go.
Broken? No. I have, however, fractured my thumb. Some friends and I were hitting some of the local parking garages with our inline skates. At that time of night, the elevators were turned off so we were forced to use the stairs for each run. The new rather large wheels on my skates came back to haunt me after a few runs. I fell on the the stairs and landed on top of my right thumb right on the edge of a step. Boy did that suck and it certainly made driving my truck interesting since it was a manual transmission. Damn thumb hurt for few months after that incident.
