Nah.

I tried that once with Mass Effect. Couldn't get the ugly bastard to look anything like me.

Even if I could get characters to look like me, I don't think I would. Watching an on-screen replica of myself getting it on with Liara would be wierd and a wee bit pathetic.

Usually I make either a heavily built white guy with long hair and sideburns or a dark-skinned chick with black hair and green eyes. Just because.

In most instances, no.

In wrestling games, my friends and I have a tradition of making characters who tangentially resemble us. For example, I end up making a "Zachary Amaranth" Wrestler who looks like me, albeit in better shape.

Pretty much everything else? No.

I play a video game to escape from reality, so I make my characters completely removed from me.
Pretty much this. Most of the games in which you can customize your character are RPG's, and as the R (still) stands for Roleplaying, I like to create a character and then play their role in the game. So to speak.

I generally try to make the main character look like a better version of me, but I won't create a clone army if I have multiple characters to create. In that case, I often base them on people I know.
