Numerous times. I care about my dog in Fable 2. Not in Fable 3 because it can never get hurt, so it might as well just be a robot.

This is going to sound like an odd one, but I'm going to have to say it was a Squaddie by the name of Soldier Nine from one of my last UFO Defense playthroughs. Why? Well, she was the ninth soldier in the squad at the beginning of the game (hence the name), and I had gotten through another 80 or so recruits before she had met her fate (the next longest survivor was the Commander, who showed up in the twenties). Watching her get shot in the back by some rookie while mopping up the last of a Battleship's crew shook me up like I just watched Mufasa get thrown off a cliff.
I renamed a base in her honour, after I executed the rookie on the spot for treason.

Very rarely do I ever get connected with characters, but there have been a few times. Recently, I was watching an old movie named Ring of Steel. The comic relief character (Brian) was an awesome guy and I was quite upset (even though it was kind of predictable) when he was killed by the big bad. He is among my favorite movie characters of all time.
I cared about pretty much all of the major characters in Final Fantasy X, especially Yuna, Tidus and their emotional attachment to one another. I'm quite sure it's the only game I've ever shed a tear to.

It's happened to me more times than I can count. Even games known for their fragile characters and weak personas I connect to. For example, I connected with Samus in Other M. Other M, the game frequently criticized for the monotone monologues that are quite dull, the game that accused the strong female character from being weak. I didn't see Samus as weak, not as a mindlessly subservient follower of Adam's commands. Oddly I rarely find myself connecting to male characters.
