Me thinks as follows:
First there is world war 3, followed by an pandemic, then famine and then anarchy ensues.
Humans as a race will survive, but our numbers will be greatly reduced, and we will be ruled by tyrants. It will be worse than the middle ages, by far.

Definitely nuclear war. Some whack job will get his hands on some nukes and actually be dumb enough to use them, thus causing a counterattack and chaining together a sequence of barrages that will reduce the world and humanity to scattered bands of helpless nothings.
Im going to go with bioterrorism. All we need is a major outbreak of ebola (or some other type of deadly disease) and we are all basically dead. It's easy. Just set it off in a major airport and watch it spread like wild fire. We humans move around so much these days it would be very hard to control it or stop it in time until it has really caused some serious damage.

but for serious. this world will never end....its the people on the surface that are screwed.
Nuclear war? governments aren't going to waste their precious nukes on civilians. each one will target a critical area to try and cripple the enemy. even if whole nations disappear there will always be far of islands and isolated villages that will remain uneffected for a time. and even if this world does become barren eventually it'll heal(40,000years give or take) and keep spinning, and its gonna keep on spinning for a long long time.
Bioterrorism? all forms of terrorism are just a form of "hyper-communication". when a government isn't listening just drive a couple of planes into some buildings, that'll get their attention
Nanotechnology? serious doubts. making a working group for starters is near impossible, even a single error will replicate itself over and over just like a cancer, except being machines they will eventually replicate to a form that has no proper function. then all you need is to play the waiting game.
Epidemic? Humans will eventually grow immune or make a cure. NO MATTER WHAT. "but what about aids?". good question! did you know there are currently 20 people in this world that are recorded to be completely immune to aids. sure its only 20 people but from their gene's a vacanine can start to be made.
Famine? more than 100,000 tons of food a day is wasted all so corporations can make a quick profit.
Anarchy? the only thing anarchy will end is government bigotry and elitism.
Meteor? not in my life time

I voted aliens. I don't think it's so far-fetched and apart from a meteor coming, the Sun going supernova or a black-hole passing by before the time we are advanced enough to get off this rock I don't think anything would do for an extermination than a good 'ole alien invasion, any other option I think at least some would live on.

I personally think it'll be nukes, cleansing the earth of humans, and giving it a few hundred thousand years to repair itself.
I just can't see us either resisting the urge to use nukes, or keeping them out of the hands of people who really want to use them, and I feel once a few get lobbed about, the tidal wave of retaliatory strikes will wipe us all out.
