I stick with normal, but if the game is too easy or if I just feel like the games too short, I put it on hard.
But almost always on fighting games I put it at the hardest difficulty, just so I could get a bit better at a faster pace.

Hard first, impossible next, and then normal to remind myself the game is actually fun.

Normal. It's how it was meant to be enjoy primarily. After that, well, nothing wrong with turning the difficulty up or down. It really depends on the game.

Between normal and hard. Usually hard unless i can't grasp the game I'm playing properly. Then its one level down. Halo and Call of Duty are exceptions. They are for hardest difficulty only. They're too easy otherwise.

I just play Easy for the story.

But, since I'm an achievement hoe, I'll play on the harder ones if the trophies tell me to.

Easy to start with, so I get to know the levels and all the little tips and tricks.
And because I'm, in want of a better phrase, a pussy.

I usually roll with normal once I know what I'm doing. I leave hard for when I really have absolutely nothing else to do. Seriously, I put trimming my nails ahead of trying games on hard.
I hate losing...

EDIT:
Scratch that, I will try games on harder difficulties if I really enjoyed them playing through on my wuss difficulties. Wet, being a prime example of a game that maintained its level of enjoyment whilst not putting it so far up that you're waiting to respawn more than you're actually playing.
On the subject of waiting to respawn, Uncharted's Crushing mode can kiss my ass.

I usually go for either hard or normal. A lot of times I start out on Normal and bump it up to hard, or vice versa.

I usually try normal, unless the game is ridiculously easy on normal, or I've already beaten it and want a challenge. The only game I've actually made this effort on in recent memory are the Halo games and Modern Warfare 1 + 2. Let's face it, Halo 3 was pretty forgiving on Legendary and Veteran on MW2 wasn't overly punishing. I never did finish my Veteran playthrough of COD4.

Most of the time I just play at the default difficulty.
Others, I prefer to crank the difficulty up from the get-go: Trine, Left4Dead, Alien Swarm, Torchlight and Shadowgrounds for example. Although, I stopped playing Hard-Core mode in Torchlight after losing 2 characters :p.

I know this sounds like a bland answer but it depends on the game. I rock fallout 3 on very easy cuz I play that game for the experiance, not the challenge, but I beat all the Halo games, both gears games, and Modern Warfares on ledgendary, insane, veteran. I also play my music games on expert or hard depending on the instrament and song.

CoD 4 on veteran is the most satasfing thing to complete, MW2 on the other hand, no challenge at all.

I usually start the game on the normal/default difficulty setting, and adjust it if necessary. After one playthrough, I usually try a harder difficulty level, if I want to replay the game. Or I play the game in casual for a bit, to admire the pretty scenery.

If it's my first time playing, then I go Normal. If it's a Shin Megami Tensei game, I stay on normal even during a New Game +. Otherwise, I graduate to Hard after the first playthrough.

Normal for most games, but if I'm playing my beloved Touhou games I'll not step a foot below Lunatic. (Well, except 10-12)
Easy for fighting games because I'm horrendous at 'em.
