Well, it's not as if the rest of the world was going through the middle ages during the era. The futuristic 60's setting can indeed work abroad. I'd hazard the guess that all western countries shared a similar ideal of the World of Tomorrow, not just the U.S. Trust me, there's futuristic Americana out here, as well. If we started comparing old, Zeerusty visions of the future from the 50's, I'd be prepared to bet that they'd match up very closely.
As for the vaults, while they are certainly a fascinating main set piece, I don't think that a Fallout game recieving the appropriate care would fail if they were included in mention only.
The trouble of a title set abroad might be to link it up properly with its American roots, true, but it's not an impossible task. Since there have been characters crossing the Atlantic, it wouldn't be too far of a stretch to have possibly Institute-personell on a diplomatic mission or a colonization effort involved.
That said, I think a Fallout game set abroad is better as DLC-material, or a little spinoff title on the side. But is it doable? With some care and attention, yes. But maybe not for Fallout 4.
In short; it's perfectly possible. But, since it'd be a different theme than the usual Fallout waltz, it'd be better suited for a spinoff or a DLC-title. The "real" games would certainly work better back home.
