Fire is really hot and tends to burn you badly on contact even through clothes. Ice can burn as well, but come on, not to the same level. Fire is also really fast and unpredictable in how it appears and spreads. It can rob you of your breath as fast as water and the smoke and flames can blind you to danger just as easily.
Well we are talking about the elements from a video game perspective. Fire (unless it has realistic spreading characteristics like in 'Alone in the Dark' doesn't pose that much of a threat more than just a wall that reduces your health. Water and Ice however change the way you need to play the game forcing you to adapt.

I may be wrong, but from what I remember they're working on nano technology that supposedly is going to make cryogenic freezing possible. Well they can do that anyway, the problem is unfreezing the people. Which is where the nanotech comes in.
So surviving being stuck in ice might be possible one day.

And if you combine that with melee weaponry (meaning close-quarters combat only) you get a situation where it is hard to get the advantage and easy to lose it. Also, ice levels normally have poor lighting and far too many corners and semi-reflective surfaces, which asdds in the element of surprise. So yeah, I'm voting ice.

EDIT: By the way, I don't find water scary, I just find it annoying.

Real spread physics for fire can really fuck you up, especially in Far Cry 2. Where water is a minor inconvenience. But games usually just specialise when they choose their "element". Bio Shock 2 is a water game, The aforementioned Far Cry 2 is a fire game.

Generally though, water will fuck you up more often than other things.

Because drowning would be the scariest thing to experience!

Water. Lingering fear from the old games where hideous, huge monsters were stuck there. I'm still afraid of water in newer games, but there's usually nothing there, or an arbitrary instakill, like in Spore. Deus Ex, Jedi knight and similar games had some freaky swimming monsters. I always keep expecting to find some huge Cthulhu-like being in Morrowind, but there's no dangerous underwater creatures, sadly, it'd fit perfectly. The water's still scary though.

Real life - I would say fire.

Games - Fire only depletes your health bar, rather than hindering your abilities to move, permanent scars and lasting, intense pain. I have yet to see a water level I like. And the music in ice levels is usually nice and twinkly. So I have to say water.

Yeah, you just have to smash a few windows to take down Rahab, but you have to bait Dumah all the way down to that certain place, and Dumah was just as effective Spectral as Material/

The Rahabim, however, made me swim so fast you could only tell Raziel had been there by the odd ripple in the water.(The Zehopnim creeped me out too, but I'm arachnophobic...)

How about all three?

In-game, usually ice is annoying, water is unnerving and tedious and fire hurts and sometimes also spreads, so fire.

In video games, I've always preferred water based levels as opposed to fire based levels, with the obvious exception of that damned Water Temple *glare*
I'm not sure why though. Maybe water doesn't seem as dangerous to me. Although it's the second most malicious thing I can think of, with the first most being darkness.

Rahab had the coolist music in the game thought.

Water. I', terrified every time I see a water level coming up. Fire and ice levels don't bother be. But ice ones annoy me sometimes.

Enh for me its water, than again I have a phobia of drowning.

I honestly get edgy watching movies or playing a game that involves being under water for a period of time.

You forgot lightning. Most animals are scared of thunderstorms, but they will go outside in the rain or snow.
Next to lightning, I believe fire is the most scary, because of it's general association with demons, hell, and rage.
