this question is easy for me, I just did it :)
Hardest acheivement to get was ending 3 on Armoured Core : For Answer.
I finally got it after countless tries (and hopeless fails)
when you get the choice, don't choose the murder choice (you'll know what I mean when you get the choice - Defend or murder with king)
I'm never doing that level again :(

I think my score is in the early 13K area.
I don't have any achievements I'm particularly proud of, because I haven't done any of the ones people consider to be noteworthy (Mile High Club for doing the last level of COD4 in less than 30 seconds on the highest difficulty, the one for killing 53K+ zombies in Dead Rising, etc).
I'm not much of a completionist.

And I am most proud of the fact that I still haven't played a game just for the gamer score. Which is why I won't ever play Avatar.

Hardest achievement? No clue. Too many to figure out, its likely one of the missions from CoD4 on Veteran, or "Scorched Earth" (complete "Burn 'Em Out" on Veteran) from CoD5.

Proudest achievement? Its the one for reaching a set speed in a vehicle with less that 5 parts in Nuts and Bolts..."Minimalist", I believe it was called. I'm proud of it because I wasn't actually trying to get it, I was merely having fun by making a "chair with a spring" and jumping around.

Memories? I recall the time I got most of my achievements, thanks to the thingy that tells you the date.

Why do people think the not killing anyone with guns in Mirror's Edge was hard?

I did that on my first play through.

Final Fight Arcade: I consistently score something like 220000 points once I kick Belger's ass out the window.

Final Fantasy Tactics: I defied the game's defiant naysaying and stripped Marquis Elmdor of his Genji gear. *collective gasps*

Battlefield 2: I once shot down an airplane with a tank while it was doing a flyby on me. I swear this is true.

Left 4 Dead: By complete accident, I got the Cr0wnd achivement, totally randomly out of the blue. I just Leeroy'd the nearest witch and blasted her face with my shotty.

Easy as pie, right? Did it with ease.

I don't know about hardest or proudest achievements.

It was nice to get Survivalist on F.E.A.R., because I got it at the same time as No Juice and Real Time (completed the entire campaign with no boosters, no slowmo, and without dying).

It was nice getting Treasure Trove on Lost Odyssey, because it took a great deal of effort for those last 20 points.

It was nice to hear the "plink" noise for beating Conan, because the last boss is basically half an hour of angry tentacle sex, with you on the recieving end. And no lube.

Me too, and I was on PC and I don't get achievements.

Funnily enough, I was just doing it because it felt better. There was only one somewhat difficult part (the part right after the Atrium on "Kate") but then I realized you can avoid everyone and just jump down (carefully) and get to the door relatively fast.

So, yeah. ;x

I dunno, I'm sure there's an achievement for beating TTFAF, so I guess that would be my most proud...

Also there's Ikaruga achievements, but they're all pretty easy, but some people might find it hard to finish a map without shooting, or beating the game with no continue.

And I think A ranking level 4 can be understandably difficult. Guess that would be my hardest (though I S ranked it).

I got Survivalist pretty easily, but F.E.A.R. is a pretty easy game to beat, especially since they give you thousands of medkits. Didn't get those other two because I was going for the achievement for finding all boosters. And I couldn't resist the urge to slow-mo shotgun enemies in the face!

My gamerscore is 11,395. The hardest achievement I've gotten is... hell. I don't really know. I guess "Zombie Genocide" in Dead Rising, but that isn't really that hard, just tedious. Before I would have said "Mile High Club" but since I've beaten that 10 times now.... And beating Halo 3 on Legendary is out, even though I had to do the entire fucking campaign by myself, but it isn't really that hard if you knew what you were doing. Oh, I can think of one! Beating Gears of War (the first one) on insane difficulty. I don't care that I had a friend backing me up the entire time, its still hard as hell.

I've got the all boosters (and the all intels) one as well, but that's mostly just a case of indulging my normal tendency to poke my nose into every corner and turn over every object.

And yeah, the slowmo shotgun rampage is pretty hard to resist.

I searched every nook and cranny for intel and still didn't find it all...And I beat the game 2 or 3 times!

My gamerscore is in the low 2000s.

Most memorable achievement would be getting the "continue 100 times" in Ninja Gaiden 2.
Since I don't hunt for achievements, the "completed level X" achievements in Ninja Gaiden are my hardest achievements to get.(I play on the highest available difficulty ad I'm not very good at playing the game)

My gamerscore 11610. I don't really care much for achievements so I'm not sure what the hardest one was.

My most proud achievement was when I shot a bunny in Fable 2. It was my first achievement in Fable.
The game gave me an achievement and the description made fun of my for it! love it.

My gamerscore is about 20500. Hardest achievement for me was getting all the diamonds on Far Cry 2 (I have limited patience) and the proudest achievement for me was the Man vs. Tank achievement legit on Left 4 Dead.

Mine is 13035 as of April 8, 2009. It will get higher. I know 2-3 people who's gamerscores are over 130,000 though so I've got quite a ways to go.

There's a couple that are a bit off the beaten track, they're all over the Armacham offices, you need to have eyes like a hawk looking out for the red flash of the phone.
