I usually play Normal on a first play through and work my way up depending on the normal difficulty. It really depends on the game, though.

Normal, and if it gets too annoying, I switch over to easy. If it's a game like...Devil May Cry or I usually start on the easiest difficulty so I can build up orbs/points/money for when I start the harder difficulties. But if it's a 3rd person shooter or an FPS, I'll keep it on Normal as well.

I always choose the default setting because it is my understanding (whether correct or not is up for debate) that games are tested and balanced for the default difficulty more so than on any other and I want the best game experience I can get as I usually only play through a title once.

Gernearlly, whatever the default is. If it gets too hard, I lower. If it gets too easy, I sometimes raise it.

I'll start on normal, but if I start getting killed within a couple of hours (which is worryingly common) I'll move down.

The only exceptions to this, as far as I can remember, have been Ninja Gaiden and I Wanna Be The Guy. I don't care if my character ends up with a bow or a ribbon, I want to finish the damn game without being eviscerated every five seconds!

I start out easy to get used to the game. After I'm used to it I tend to make the game harder.

Depends. If I'm generally good at the genre or at least have played enough to know basic controls I play on hardest (so usually hardest). But if a genre that I'm not so experienced with, RTS for example, I'll go normal.

Normal. I want to enjoy the game the first time through, when I go back to play it again I'll chuck it on a higher difficulty so it has better replay value.

I'm pretty confident in my FPS skills. So I usually go with Hardest in those. On the PC mind you.

most everything normal to normal-hard. rts though... easiest possible. i can barely win on that mode.

Normal. Easy to change directions when you are on that setting. Usually I stay on normal (or medium) but occasionally I'll say "Bah, lets kick this up a notch." or "I feel like I'm doing this wrong, lets take it down a notch."

FPS games are definitely hard to hardest... my first play through of both Modern Warfare games was on Veteran.

most everything else, normal

Shooters and RPGs I will often just start on Hard these days, however thats simply due to regenerating health and the such which make newer games much easier than the older ones.

Everything else I just start on normal.. not to easy, but not so hard that screens and controllers get broken during fits of rage.
