Of the non-lethal ones probably cluster headaches, since there are people who've experienced several of these and still said the headaches are easily the worst to the point they can make people wish they were dead. I think being devoured by ants would probably be even worse, though.
I've only ever personally experienced animal venom and sledghammer to the balls, so it's hard to compare. An ex girlfriend once had me take her to the hospital for kidney stones, and it looked like it hurt something awful. But I can't really judge from that. Animal venom... it depends on what animal. I've been bitten by so many recluse spiders that all it does is kill some skin and leave a brown spot... hardly hurts at all. Scorpion sting on the other hand... got infected and I was down for a month. As for the balls... wasn't a sledge to the gnads, but a full hockey puck slap shot. That hurt far worse than either venom, but only for a day or so.
Although I wouldn't argue childbirth with any woman who's gone thru it. Newborns really aren't that small. If it means I have to concede the "most pain" win... it's all yours.

I have precisely fuck all experience of any of these things to draw conclusions from. I've heard Platypus venom is one of the most abhorrent substances know to man, but to be honest, I think being eaten alive by ants would be worse. Even with millions of them it would be slow. Stripping away your skin and flesh piece by tiny piece. Thousands upon thousands of sets of razor sharp pincers munching on every nerve ending in your body in unison. I really can't imagine a worse kind of hell.
I voted for sledgehammer to the balls, but I really think the burning to death would be the worse, if you died from the flames and not CO poisoning. Being a firefighter, I've seen it, and it is really bad.
I mean, burning to death would suck but I don't think you'd be alive that long but being eaten by ants sounds... slower.
Then again bleeding out from childbirth and dying that way doesn't sound like a field day.

That said, I voted fire. I once got a terrible, terrible fever overnight, while under three blankets, on an unexpectedly warm night... and I dreamed of burning to death. It was far and away the most awful dream I've ever had by an enormous margin.

But I personally do not think of (even) the Bullet Ant, rather of the Black Widow... it's not rated in the Schmidt Sting Pain Index because it's a spider and not a hymenoptera, but it's said to be the most painful thing in the world. A poison which directly attacks your sense for pain. How can there be something to be feared more?
Childbirth, but only because I'm not sure it's possible to be eaten alive by ants. Most ants generally wait until something is dead. Burning to death often just leads to suffocation, though it is still incredibly painful.

Actually though, nevermind. This is "worst" not "most painful." I'm changing my answer because at least most people don't die from childbirth. Childbirth is something that people routinely see as worth the cost, whereas burning to death... not so much.

Really? I always heard that it was Stonefish or platypus venom. There's also a species of millipede with a sting so painful that people have been known to plunge their hands into boiling water to ease the pain.
